Rapid Coronavirus Testing - At HOME (COVID-19 Antigen Tests) with Dr. Michael Mina

  • Michael Mina, MD, PhD responds to questions about how cheap (approx. $1), at-home, COVID-19 tests (results in 15 minutes) could be utilized to dramatically slow the spread of this pandemic (and open up schools etc. in a faster and safer way).
    Dr. Mina is an Assistant Professor of Epidemiology at the Harvard T.H Chan School of Public Health and a core member of the Center for Communicable Disease Dynamics. Dr. Mina's research has demonstrated that the sensitivity of these simple daily quick tests (the technology already exists) is high enough to detect the vast majority of infectious COVID-19 and could be utilized frequently at home with a nasal or saliva sample.

      I would buy 1000 PRIVATE rapid saliva covid home test TODAY, like pregnancy and HIV test I can do at home today.
      A pregnancy test that tells a woman she is pregnant 10months later is useless.
      A thermometer that tells me I had a fever 3 days ago is useless. A covid test that take 3 to 7 days is useless.
      Both the CDC and FDA need to explain what the delay in rapid testing was and Why there are no cheap PRIVATE test available yet? We can save lives with early testing, and early treatment.
      CDC received notification of the first case of laboratory-confirmed COVID-19 in the United States on January 22, 2020. As of today it's been slightly over 7 months and we do not have better means of testing?

      @A P You're right, but it sounds like they have serious accuracy problems. Better than PCR tests, though, just for speed and cheapness.

    There is two really easy and dirt cheap way of solving this.
    Make the test part of a QR code. the phone scans it send the information to department of health, that in turn returned the test result.
    The other way be simply to deploy it in a other country.

      Excellent idea. Send them here to the Philippines. Our government is doing a horrible job returning test results. Some test results here are months old.

    In an emergency, it should be possible to SUSPEND an FDA rule with just 5 states voting to do it.
    We do NOT have a free market. In a free market, this would have been done in February.

      Probably because a pregnancy test, if it produces a false negative, won't make others pregnant. Don't get me wrong. I think these tests are a great alternative.

    The CLIA regulators reminds me of the history of “fecal hemoccult” testing. Before CLIA, we (General Surgeon) did our own fecal hemoccults with every rectal exam. And almost everyone got a rectal exam. Thousands of fecal hemoccults we’re stopped by CLIA because Physicians were not licensed by CLIA (test not reliable in MDs hands: very low sensitivity ). Thus, now missing possible occult bleeders, even though the test had very low sensitivity. Then, again because of very low sensitivity, the rectal exam is no longer routine. Widespread use of a very low-sensitivity ( but free) test will pick up the rare exception to the rule, and hopefully early.

    Think about this. Every kid entering school could be tested at the school, or every person attending a wedding, funeral, etc could be tested at the door and know if there are any risky people before anyone enters. Every employee at a hospital, senior living facility, etc, could do the same. Many fewer would be missed, at a pretty nominal cost.

      Sorry, One virion is not sufficient to cause illness, as is the case with most infectious diseases. Only Hepatitis B is known to cause illness with only one virus particle. One has to have enough virus to be able to give enough virus to another person to call illness. Immunosuppressed or immunocompromised people need smaller inoculating doses and get more viral replication more quickly.
      If this works, do you care if someone makes a profit? Do you care that Steve Jobs made a bucket of money on smart phones if it made life easier for the rest of us? I do have problems when government tries to pick winners and losers-like Solyndra. We shouldn’t be on the hook for shysters. They should just allow competition and monitor whose tests are better.

    Change the designation of the low-cost-self-administered-procedure. Don't call it testing. Rename it to something else like, covid-eval or viral-load or something else that would take it out of the preview of the regulatory agencies who are stifling the implementation process.

      Mina is a political activist on the pro-Dementia Biden track. Trump would not / should not trust such a person.
      Where are the demonstrations of these tests? Mina claims these tests have been available for months. Proof? The theory he propounds is perfectly reasonable. But, that's abstraction. Before launching concrete testing programs, concrete proof of test efficacy is necessary.

    Thanks doc for your fight. Iam a school nurse who is exposed to sick children all day. I took home a selftest kit and tested positive. I took my 5 day medication regiment and on my way back to work.

    Ok, like many others, I have emailed, tweeted, and otherwise driven everyone I know crazy spreading the word. Luckily I have watched every Medcram update on SARS-Co-V-2 and feel fairly proficient at explaining most of this "stuff " (that's a medical term). My state legislators have also been notified (tho, I doubt they really listen to constituents) as has the president. Any other suggestions?

    The FDA just removed the requirement for EUA for any Covid19 Antigen test as of Friday August 23. That should really speed up distribution.

    Astute, clear presentation, educational for the cardiovascular scientist I am. Dr. Mina is conveying a strong and convincing message to the FDA and the Federal Government. If more scientists think the same as Michael, then they will have no problem gathering momentum and getting the message across. Hopefully, we will get the rapid home test that will slow if not abolish the transmission of covid-19.

    It's really become a marketing issue. While what Dr. Mina and the other experts are saying is completely correct, the message hasn't translated well into what less sophisticated folks understand. Perhaps it would be better to present these types of tests as a screening tool, similar to other test strips. For example, if one uses a diabetic test strips and gets a positive result, one acts to lower one's number. If a pregnancy screening comes back positive, it means a trip to the gynecologist. If you get a positive result using these test strips, you don't go to work or school, but call your Dr. The idea needs to be simple, to be understood; the fuller science can follow, after the basic idea/intent is understood.

    The listed names almost certainly have working C19 test strips in their labs. At least one them has foundations in pregnancy test strips. They'd rather monetize proprietary "espresso machine" versions thus little incentive to promote cheap, low margin test strips. It follows that running interference at the FDA should not be surprising.

    What Dr. Mina is missing from his viewpoint is an awareness of the WHY for regulatory processes. Like the vast majority of scientists and medical practitioners with "a good idea" they simply view it as "bureaucracy" and do not take the time to learn HOW to navigate what is a very stable, very repeatable process, nor seek guidance from a qualified consultancy to help them navigate (rather than having to learn the regulatory paradigm).
    This is hubris, pure and simple: My idea is special. The times are special. Ergo, I am special. And can ignore the very system that prevents dangerous or unreliable product from making it into the marketplace. Because this is special. Myself and my colleagues in Regulatory Affairs (yes, the people that help you navigate) have seen this behavior A THOUSAND TIMES. it is not new because we're dealing with coronavirus.
    I will guarantee that he has not done the same due diligence on getting advice for achieving agency influence or audience, that he does for ANY scientific research project. These people seem to think that the scientific method stops with them and their work and somehow miss the fact that the agency, which has the best reputation for medical product oversight in the world, operates on those very same principles. It's just a different context than a peer-reviewed journal environment. A little bit of respect and curiosity could go a long way here.

    But FDA nor anyone else in officialdom was remotely interested in HCL early as a zinc ionophore as was outlined in your no. 34 - and which Dr Zelenko has recently said was the inspiration for his protocol - and also which looks to have dramatic benefit - so why should we expect them to be any better on easy testing?
    It's like with Zn+HCQ (+AZ) - we're left with Frederic Douglas' saying 'Power concedes nothing without a demand'.

    Nothing is more important! I look forward to these tests being in the hands of every American to use at home every day. Combined with masks, all schools and businesses could immediately reopen. To remove incentive to lie about test results, we must also provide paid sick leave so those with positive results can afford to stay home.

    This approach appears to be useful for random/serial testing with high environmental prevalence. Yet in the context of contact tracing, you put all persons at higher risk into quarantine (no need for quick results, although speed increases compliance) and you're especially interested in those that may have been infected just 1-3 days ago (the edge of the outbreak), with a low probability of being infected (thus not immediately quarantined). These are not infective at present but will be in the future. You need to catch them in order to put out the outbreak, and you need sensitive test to find them. A Bayesian analysis on the specific role that inexpensive, less sensitive test may play in that scenario would be important, as the random/serial surveillance testing scenario would apply primarily to an out-of-control situation that should actually be addressed by a temporary but stringent shut-down.

    I have spoke to a few folks about the potential for this method of testing . This really is quite impressive and paradigm shifting . I'm really hoping this take off with vigor . I'm praying that many many folks don't have to go through what my family is going through . This would help that fight tremendously . It could be given to schools and employers /agencies for testing students , teachers and employees .
    The big problem is this test has not achieved public and agency knowledge of usefulness application . This test would allow schools nationwide to open if implemented nationwide . Each school would have to test each morning , each student . Line kids up , spaced out , paper test , wait ten minutes . Suzy hits positive . Shes sent home and school reports test to local health authority on that individuals results . She is then retested by agency until she clears and then is allowed to return to school . Spread is slowed in schools . Walmart , Lowes , Frys etc , tests employees each day with government paid for tests , daily . Expensive ? No doubt in those numbers . But it's far far less expensive than shutdowns . It would prevent small and medium sized companies from needlessly being closed . Same for bars and similar facilities . Bars need to be testing patrons at cost to patron . If patron wants to drink in bar , then break out 4 dollar test fee .
    Tests simply need a generated bar /qr code , subject fills in personal data before test is performed . Companies false reporting negatives get hit with very large fines . I can't imagine a company or school reporting fake negatives as it would put them and their own families at grave risk . But I'm always suprised by humans .

      Testing at workplaces and other public places could be used as a way of registering positive cases with the public health department. Perhaps they would allow this cheap, quick saliva test this way.

    Seems like another test analogy would be ketone test strips or diabetic finger prick tests. Both can be done at home, neither reported to the state.

    Are you aware of either Avacta's or Sona's rapid antigen lateral flow tests that are currently undergoing clinical validation? Sona are using nasal swabs and avacta are using saliva samples, both are partnered with Cytiva.

      I googled it, and all I could find is an antibody test.
      Antibody tests tell you where the virus was weeks or months ago. They're useless for surveillance, or for preventing new transmissions.

    Hey guys, here in Melbourne Australia we are once again in a huge lockdown with curfews for just a few hundred cases a day. We test daily around 30,00 people however as you've said most people are waiting 3 or 4 days to get tested and another 3 or 4 days to get results. Could Dr Mina chat to Australian health officials with this amazing testing idea and how it will quickly stop community transmission!!!

    "There doesn't seem to be a will to bring it to market". Same goes for cheap therapeutics. Why do we have to wait for countries like Iraq, Iran, Costa Rica and Malaysia to tell us whether Ivermectin, Doxycycline and Bromhexine are effective? Where are the RCTs? Western countries are patiently awaiting arrival of costly medicine for critical care patients... Whereas developing countries are testing cheap existing medicine that works on early infection phase.

      It's so much easier in smaller countries to see what works. That is why this should be started within smaller groups to see if it does work when everyone is being cooperative. If it works in the best case scenario then it could be expanded.

    If a smartphone was part of the test it could also report the results over the internet to meet FDA regulations. Not that I agree with the FDA regulations.

      @@fewferfev Yes, this a suggestion on how to satisfy the perposed FDA requirement for reporting test results to public health authorities and the test results are supposed to be more accurate than a human examining the test strip themselves. I would still prefer a simple paper strip test.

    One problem will be cheaters - no more sick time, can't stay home with sick kid, etc. But audit protocols based on QC sampling at entry points can help. Sample x employees/students at entry points. Businesses will have to have Covid absenteeism policies, e.g., PTO for verified +strips, etc. Have answers when the naysayers / vested interests jump in.

    I'm writing this before watching the whole video because I don't want to take a chance of comments being "turned off". So maybe I'll be able to answer my own question. Yesterday our Governor of Ohio, Mike DeWine, traveled to Cleveland to greet President Trump. For some odd reason he wasn't tested prior to going. He was instructed by The White House, per protocol, to take "the rapid antigen test" when he arrived. Minutes before meeting with the President he ended up testing POSITIVE and had to turn around and go back home. Was on national news. Journalists didn't give details of exactly what test it was, just that it was very rapid, so I'm thinking could be what you are speaking about.
    Well, A FEW HOURS AFTER learning he was positive, another announcement comes on air saying when he arrived home he went to OHIO STATE UNIVERSITY Wexner Medical Center and took an RT-PCR test...which ended up being NEGATIVE. We don't know what to make of this. Most sources are saying the rapid test isn't as accurate so they are dismissing the positive. It's my understanding though that the rapid antigen test, while yes it doesn't always detect the virus so has many false negatives, does not usually give false positives. What is your take on this? Many confused people in the general public of our GREAT BUCKEYE STATE, many of whom are about ready to give up on the whole thing.

      BTW....he said even though the 2nd test was negative he isn't taking any chances and still quarantining until he has another test, which I think is wise. I'm an Independent who tries to see things as neutrally as possible and have to say this particular Republican Governor cares about his people. He was one of the first to shut down bars and other non-essential businesses in March then gradually reopened, emphasizing taking precautions and recently mandating masks since crowds were getting out of control and not social distancing. He has kept us informed almost on a daily basis which i appreciate.

    a better analogy to gas lighting and major fire would be a pan fire and a major fire. Lighting a gas fire is a daily event with no significant risk where as pan fire is more serious but can be managed well in most cases.

    Why not sell it as a pseudo-supplement? We see a lot of those in the market and with a tagline of "No therapeutic proof and not approved by FDA!". Still, people will buy these regardless of what FDA says.

    How many Days is the window of transmissibility that Dr Mina talked about? How many days can an infected person infect another one? 3 days? 5 days? A week? I am not asking how long to quarantine, but how many days the latest science says that a person can pass the virus to another person. The Medcram graph talks about CT counts, but not in terms of days.

      I went to the rapidtests website and used method 2 to contact two Senators, a Representative, and a Governor. It was pretty easy. Method 1, which uses a platform called "resistbot", may be easier but less effective.

    Has anyone attempted to promote this idea directly to President Trump or VP Pence? It may be an obvious question, but the fact that he has multiple times brought up out of the box ideas, as well as overriding CDC regulations, I think this sort of change to testing would be considered. Going through the FDA seems like a dead end as they don’t have a directive to approve public health tracking.

    Very good idea with cheap and affordable test kit at home. We live in this country with @350M mixed of races, cultures, religion, etc. even if we have all the test kits and vaccines, that does not mean everyone wants to take it, it likes "you can lead the cows to the water, but you can't make them drink.", right?
    Also there are technical difficulty details like you mentioned is how the device report/send result to CDC or healthcare system, which could make the device from cheap to very expensive. Right now almost every country around the globe are working on test kit, vaccine, and treatment, including those fake ones who want to rip off poor people with faulty device (try stuff Made in China and see), thus the FDA needs to regulate and prevent that scenario from happen as much as they can and most of all for the safety of people. However, of course, we always hope there will be the light at the end of the tunnel, with our modern technologies and our scientists working hard day and night, hopefully solution to those issues are on the way in future or sooner.

    I listened to the live stream. I 100% agree there needs to be a cheap, easy to administer, accurate test which can show whether or not a patient (human or other animal) is actively infected with cv19. But, there was too much speculative science and not enough solid deliverables in this lengthy video stream. It was pie in the sky. If a small lab has such a product, then yes it should be pushed thru at the highest levels, but all I heard is maybe, could be, needs a little more work, etc. Did I miss something?

      Randy Rick, well no matter how hard people are working to track and trace after PCR testing it’s almost playing impossible catch up with ballooning numbers of community spread. The time delay regarding results is too lengthy despite the labs working 24 seven shifts and increasing the staff dealing with Covid 19 tests only. This test would be very helpful to help with implementing Pandemic control tools available without having to resort to lock downs again. People have become so impatient awaiting their test results and many aren’t isolating or staying home from work until they are cleared, that’s very problematic as many do record positive results and those exposed multiply hugely and then voila! out of control contagion full hospitals and over worked front line medics, police and very sick or dead patients not to mention the risks to front line medical employees. Australia is talking about similar day tests too I feel that they will get their use happening to avoid the current situation happening in a major State there. Big Pharma doesn’t influence Australia to the same extent as it does in the USA with their PBA system good cutting edge treatments are affordable to anyone requiring them.

    They say they have a high failure rate. So...they're cheap. Do 3 and make 2 of 3 a Positive.

      The problem is, they don't deliver false negatives at random. The primary problem is that when a sample's virus count is low, none of the three would go off.

    Interesting stuff but ... this virus cannot be stopped, only slowed. Right now with all the restrictions in the UK general health has very much declined in many ways ... restricted access to healthcare for other conditions (e.g. cancer screening), closure of gyms (just reopened but with alot of limitations), immune system not exposed as normal, increase in obesity through less activity and worse eating habits, mental health issues due to isolation, domestic abuse increases, job losses causing poverty, etc.
    We need to get to herd immunity as quickly and painlessly as possible. Perhaps the testing can help with this but I fear some will leverage it to force fascistic lockdowns which are counterproductive.

      Echo - I doubt herd immunity will happen. Aids eventually is treated, and other diseases, such as CLL SLL has treatments. I think the focus should be several inexpensive treatments, not a money maker for pharmacy.
