    As a radiologist in training I must admit: This is one of the deepest videos I have seen in a while. Also chest X-ray order that just says "chest X-ray" is a thing.

    After a car accident the radiologist saw a bullet. I was born in and lived in the USSR until it fell. I had honestly forgotten about being shot at that point and they came rushing in desperately looking amongst the blood for an entry wound. After much thrashing me around someone thought to include me I'm a conversation and I let them know I had been shot when I was 6, its no big deal.
    For an American hospital they seemed weird about a 35+ year old bullet and where it was from. I got exceptional care for my actual injuries though.

    ER DOC HERE ! This is very true !!! SOO Accurate ! Radiologist NEED a story that way they know what to look for and where to look and they will be able to provide a much better report. I do my best to let them know. Is NOT an easy job :/.
    I do see my patient first ! :)

      Rad/CT/sonographer here…words can’t say how appreciative we are of docs like you, especially at 0200! As retired military I’ve worked many places and though some facilities required the referring physician to contact the on-call radiologist first, many don’t. Having a thorough history really helps us to get what the radiologist (and you) need for a prompt and correct diagnosis.

      @@hinotefanatic Wow ! Thank you ! Yes we have a section on all our orders where we are supposed to write that info BUT yes some just put pain, or STAT. And you guys are the experts at getting the images (Gallbladdder,etc) STay safe

      When I get scans in the ER, the techs always ask me "what's hurting, what happened," and all that. They aren't the doctors but I feel like all the parts are being educated to get the info and hopefully it matches up in the end. I did work as a secretary in an ER center court back in 2007, the stories rarely match up 🤪

      Damn. Ortho docs notorious for leaving us rehab (OT, ST,PT) in the dark about their post-op recs and weight bearing etc. like would it kill y to at least include weight bearing!? Hahaha! Also love it when they write turtle shell back brace “for comfort” 😂😂😂 ummm I’ve yet to meet the patient who wants to wear one of those things let alone for “comfort”

    Thanks for a good laugh this morning doc! As a radiology coder, this sort of thing is the bane of my existence! Every time I have to code a report with no findings and vague, borderline unusable indications for why the exam was done in the first place, I die a little inside. I'm begging you, please tell me exactly why this patient needed their left foot x-rayed so insurance won't deny it as unnecessary 🥺

      We don’t know what the X-ray says until radiology reads it to find out if it was necessary or not.

      @@dr.floridamanphd The purpose of the x-ray is to help you answer a question. Explain what the question is that could be answered by an x-ray and you have justified the x-ray, it is really that simple. If you can't formulate the question, chances are that the x-ray is not justified, and you should re-think your choice of tests.

    I remember this! Since I worked weekends I had the unpleasant task of telling the ER docs that stat or pain was no longer an indication for a CAT scan or an x-ray. They had to give a some kind of history in order for insurance to reimburse. You would’ve thought that I’d ask them to beat their wives and children! Oh the memories!😂😂

    Being a Traveling Radiologic Technologist I can say this is very true. I often feel like I should be dressed as the Gorton's fisherman instead of in scrubs because the vast majority of CTs are frivolous and essentially a fishing expedition. It does make me wonder if ordering physicians half ass their job when ordering medications or just when it comes to Radiology orders, especially from the ED because I'll routinely receive orders to inject contrast to patients who are allergic to it; often never seeing the patient, let alone performing their due diligence in checking the patient's chart.

      I once worked for an ER Doctor who would let the nurses order all the tests for the patients. He would never look at the X-rays. More than once I had to walk up to him with an x-ray and say “hey Doc what do you think this is?”. He would scream “oh my god where is the patient?” and I would reply “being discharged”. Fun times….

      Yeah. I should have been a radiologist for sure. I'm "dark adapted", and I get comments all the time, even from family, like, "Why is your living room so dark?", or, "It's like a cave in here. Why don't you open your curtains and let in some daylight?" That one from my daughter's father. I swear that was one of the biggest reasons we didn't stay together.😊 Well, no harm done. I wouldn't want to be married to a non-dark adapted "Daylighter."😅

    Hey! As a radiolographer, I can say that there are also requests for repeat x-rays without any indication if they did anything to the patient, note that most of the times we just took an x-ray of the patient the same day 😂

      Hey as an icu doc, we look at progression as the grey areas getting blacker equals good and grey areas going white equals bad. So if radiographer has got exposure settings garbled you're gonna get a repeat cxr as I don't my good progreasion as demonstrated by the levelling out tlc counts.

      ​@@nhilistickomrad4259that's all well and good but they're stating about the fact there's no clinical history relating to a repeat. If you're aware of radiographers then you also are aware that under IRMER guidelines you are required to give correct information relating as to why you're asking for the xray. All it takes is a quick note saying "reviewing consolidation, ?resolution" a Radiographer rarely dicks around with the exposure factors especially within enough of a range to completely screw up the contrast balance

      @@vickyvkusnyatina6126 So ordering STAT for anything requires a review & consult by a radiologist, and only approved if absolutely necessary. They do not like to be disturbed if not deemed a true emergency.

      Tbf as a Radiographer SOB is a valid indication for a chest xray. It's absolutely the bare minimum but it can be justified. My favourite is "CXR please thanks" with no other indication or "portable please" with absolutely no clinical history. Like is the patient coding or are they walking around with nothing wrong with them... Which tends to be the case

    Lol I’m not a dr but I worked as a scheduler in a radiology center… reading orders from drs and I relate to this so much. Ortho always ordered STAT even if it was for something that could be scheduled later 😩

      Disclaimer: Not a radiologist but I am a radiographer. "Pain" isn't a helpful clinical indication. We need to know where the pain is. How severe is the pain? How long has it been painful? Is it due to a recent injury or has it been there a while? Does the pain only come when doing certain activities i.e. walking? That sort of thing

    OMG, spot on with the ID H&P's. My hospital's ID doctor is one of the most thorough, and I love to read through his patients' H&P's to find a full family history dating back to the 14th century.

    Fucking yeeeeees. I'm an x-ray tech and the history for any view is just "[insert body part] pain". Then they get mad when I don't do the special views that they didn't ask for. Props to all of them for even putting the order in before sending the patient over though.

    As a CT/xray tech, this is on point!!! ER is terrible about not seeing the patient before ordering. Ortho-"you had an xray 2 days ago? Let's get a repeat" or STAT CT, but we will see patient next week... Our infectious disease indication is always "fever unknown origin"

    I have never wanted to be a doctor. Having an academic career never interested me. But I love being on the outside looking in laughing at your videos. I find that world intriguing enough to read novels watch shows and laugh at videos on the internet.
