Thanks vincent, are you saying we should start with the observations first e.g bp, pulse , temp etc before the neurological assessment? The GCS chart statr with the spontaneous eye opening, verbal response before the vital observation. What do you suggest? Follow the chart systematically or start with vital signs observation

Hello Vincent . Please enlighten me regarding the pain of the patient do we need to continue the observation without giving analgesia even the pain is high scale?

Yes. Any medications need prescription so you will tell the patient so and do whatever can relieve the pain at that time eg positioning. Then proceed with the assessment. You have also reassure the patient that you will check their prescription chart for any due analgesics.

You will verbalize to the assessor that the state of the patient doesn't allow so you proceed to using the paperwork and wrist band
