From what I heard back when I was collecting Mayhem stuff the guys who sent the yellow coupon back to Euronymous didn't get anything in return for it (apart from a personal letter maybe). Apparently no lyric sheet or anything was ever made.
Can't say I'm surprised. Some dude I know in person sent him $$ for the Merciless LP and never got it. The good old days where you would stuff money in an envelope and wait months hoping something would eventually turn up in the mailbox while having a 50:50 chance of ending up being screwed.

I know two guys one of them bought the death crush at Leipzig from the band themselves and it didn't have any number and the other guy copy didn't have euroymous name

Could never get into Burzum just due to the fact that he killed Euro... Had he lived, it would have been better for metal IMO. Better than what we got from Varg.
Well then by that logic you may as well stop listening to mayhem too since Necrobutcher admitted in an interview that he would have killed Euronymous if Varg hadn't gotten to him first. Euronymous was a piece of shit (surprise surprise a black metal musician is a piece of shit) that made many enemies after he encouraged the suicide of Dead.

Don't know what you're talking about man. It's clearly stated the original came out in '87 and the DSP version in '93.
