  • Facial pigmentation can occur due to various skin conditions. It can be distressing for patients and frustrating for the treating medical professionals. Oral tranexamic acid has been used "off label" to treat pigmentary dermatoses that affect the skin. This video looks at the conditions that could benefit, the dosage used and potential side effects.

    I’m glad I found your page. I have post inflammatory pigmentation. I had pityrious rosacea. I have now dark spots everywhere on my body. My skin was so beautiful before that my self esteem has declined tremendously. The determalogist apparently was not use to treating African American skin she had me soak in bleach. It made me sick.

      It can be difficult to treat post inflammatory pigmentation. Moisturising is important as it gradually helps reduce the pigmentation.

    Hey doctor! I absolutely love the valuable information you provide through your videos. I have been struggling lately with my skin. I'm Indian, brown skinned, no medical issues, insulin is alright and no genetical abnormalities. I got bad tan on my neck, and my torso is relatively few shades darker than rest of ny my body, my armpits are also heck darker. Can i go with tranexemic acid 500 mg? Recently I injured my skin as well from exfoliants. Really need your advice doc

      There are numerous treatments for reducing pigmentation of the skin of which tranexamic acid is one. If pigmentation is prominent in the flexures, another possible differential diagnosis is acanthosis nigricans for which met firming may be helpful. It may be useful to see a local dermatologist to get the diagnosis confirmed first. Here is a video on metformin for acanthosis nigricans.

    Hi Doctor, I thank God that I landed on this video. Very nice Information and Thanks a lot for sharing. I am in India and 55 years old. I was hopeless due to my skin condition. I used Diprovate MF for a long period. As the medicine is no longer available , I have a serious skin problem. I can see similar skin conditions in the video. I tried homeopathy,ayurveda but no use. Should I take Oral Tranexamic acid?

      Thank you for your kind words. The first step before any treatment is to get the diagnosis correct. For this, you will need to see a dermatologist. If they feel you have a condition that may benefit from oral tranexamic acid for pigmentation, then it is worthwhile starting. Also to prevent the existing skin from darkening it is worthwhile using a tinted sun screen. Here is a video on it

    Hello Doc, I am a 47 years old male with no health issues. I just have a small melasma patch on my left cheek. Can I take Tranexamic Acid 250 mg pills twice a day for 3 months to see if they will work. I have tried tooical alpha arbutin and Tranexamic acid lotions but no luck so far. Thanks in advance.       Thank you for your kind words. Firstly, the diagnosis of melasma needs to be confirmed by a dermatologist. There are other causes for pigmentation on the face. These include maturational hyperpigmentation and also secondary top hair dyes. Here are videos on each of these conditions.

      If melasma has been confirmed, then tranexamic acid is a useful medication. It is also important to use tinted sun screen to prevent worsening of the pigmentation. Here is a video on it.

    Doctor, I am female, living in the UK. I saw a dermatologist a few days ago and was prescribed oral minoxidil (half of a 2.5 mg pill a day) for androgenetic alopecia. I asked about Finasteride as well, but was told that they will prescribe it only if Minoxidil doesn't work). Do you think it's right? I am tempted to use an online pharmacy and ask them to prescribe Finasteride as well as I doubt that Minoxidil will work on its own, since it doesn't reduce DHT.

      P.S. I am 44 and started to lose my hair about 1.5 year ago. I haven't thought anything about it at first as my hair was very thick, but my hair loss has been progressing...

      We generally try and use oral Minoxidil on its own as it is fairly effective. Most dermatologists would feel uncomfortable prescribing finasteride in women, so I can understand why your doctor is hesitant to do so. One option to control the hormonal component may be to use spironolactone. We use it quite a bit for acne I women and it can be used in combination with minoxidil for hair loss too.     Can Tranexamic Acid 500mg and roaccutane 60mg bd, be used to treat lichen planus pigmentosus?
    My dermatologist prescribed that for over 3 months.

      Yes, it is a combination that can be helpful for LPP and dermatologists in Asia use it often. The most important step is to use tinted sunscreens that will help prevent worsening of the pigmentation. Here is a video on it

    I developed severe hyperpigmentation after a chemicall burn, all over my face, its been 2 years the pigmentation slightly lightened but for the most part it is visible, can i use trenaxemic acid and see changes?

      Tranexamic acid has been used mainly for melasma and other conditions mentioned in the video. It’s probably worth trying in other conditions like mentioned in your question. Tinted sunscreens are also very important to prevent further worsening of the pigmentation. Here is a video on it.

    Hello Doc! I was told that it is not safe if we have clotting disorder. How do we know if we have a clotting disorder? Is there a test for that?

      Tranexamic acid has been used to treat some clotting disorders but may need to be avoided in others. There is no specific test for it, but an assessment by a haematologist could be considered if there is a pre existing clotting condition.

      Tranexamic acid has been used mainly for facial pigmentation, so it's hard to know if it will help post KP pigmentation. Tinted sun screens are probably important to prevent further darkening of the skin. Here is a video on it

    Hello Doc! I am taking Antoxid HC (30 mg beta carotine) daily and it seems to work on giving me eventone and glowing skin. I am a non smoker with no health issues. I was wondering if it is safe for long term use. Please, advise.

      I do not prescribe beta carotene often so not sure of its long term side effects. It has been used in photosensitivity disorders for many years, so it is possibly safe as I am unaware of any significant toxicity with it. Lifestyle measures may be the best way to have healthy skin.     Hi,
    I have read up reviews stating that tranexamic acid does help with pigmentation however it causes severe hair loss. And also leads to greying of hair as it hinders melanin production. Is that true?

      Any drug can cause any side effect, but I am not aware that it is a prominent well recognised side effect of tranexamic acid. It may occur but is possibly rare.

But just wanted to get a medical insight regarding the thought that if something hinders melanin production is it likely to cause greying of hair as well? And if thats true the greying could also be caused by oral glutathione?
      Also please let me know which one is more effective oral glutathione or tranexamic for melasma?

      Its usefulness is more for pigmentation rather than epidermal changes that occur in acanthosis nigricans. Metformin may be a better option in acanthosis nigricans even in non diabetics. Here is a video on it

    Hi Doc, I live in Canada and we can only get Tranexamic Acid tablets in 500 mg pills and they are film coated. Can I split the tablets or should I get sustained release 250 mg tablets from India? Please advise.

      Yes, it will be fine to split the tablets. Ideally the 250mg tablets form India may be the best long term plan.

    Hi Doc, if I may,
    with reference to a comment here, I would like to ask which of these combinations would be the most effective to treat LPP for an Indian:
    1. Tranexamic Acid 500mg + Roaccutane 60mg
    2. Tranexamic Acid 500mg + Glutathione L 500mg
    3. Tranexamic Acid 500mg + Roaccutane 60mg + Glutathione L 500mg
    The 2nd one was suggested by the dermatologist first. Later when I quoted this comment, he said I could add Roaccutane to the other 2 drugs.
    Kindly advise doctor. Thank you.

      Hi, I do not treat many patients with dark skin as I work in the UK. From all the lectures I have attended and the personal expetricne in having treated a few patients, a combination of drugs may work better. The dose of Isotretinoin (Roaccutane) used is much lower though, about 10 to 30mg. I do not use high dose isotretinoin as it is likely to cause more side effects. Most dermatologists who are considered experts in treating LPP in dark skin suggest tranexaminc acid with isotretinoin. Glutathione is an optional extra

      If the rail function is normal, most drugs are safe to use. Tranexamic acid has been used for at least 3 months in most studies and may have to be continued for longer in some.

      Oral tranexamic acid is usually used for pigmentary problems. For skin barrier issues, a good moisturiser is required. This video looks at how to treat sensitive facial skin. Though it mentions rosacea, the principles are similar for anyone with sensitive facial skin.

    Sir I had cyst couple of years ago and right now I am thinking of taking oral medication for my hyperpigmentation. The hyperpigmentation is all over my body and severe around folds. Please do suggest what can I take without much side effect, if not tranexamic acid due to the history of cyst.

      The most important step is to identify the cause of the increased pigmentation. You will need to see a dermatologist for that. A common cause of hyperpigmentation in the flexures is acanthosis nigricans and that requires another medication called metformin. Here is a video on it.

      it depends on the underlying condition. If it is post inflammatory pigmentation, the condition may not recur. Other inflammatory conditions like lichen planus and melasma may come back . One treatment that may prevent pigmentation or at leats prevent it getting worse is the use of tinted sun screens. Here is a video on it

      Tranexamic acid seems to be beneficial for pigmentation of various causes. It may be worthwhile consulting your own doctor before commencing it. Certainly a worthwhile treatment to consider.

    Hello Doctor! I'm a 19 years old female who has been diagnosed with post inflammatory hyperpigmentation all around my lips in big patches. I've taken Zimax antibiotics for 3 months and used azelaic acid 20% for 2 months but still no improvement. Can I take oral tranexamic acid for this?

      Tranexamic acid has been reported to be helpful in post inflammatory pigmentation, so may be beneficial in that setting. There are other topical agents that could be helpful too.       There have been no studies on oral tranexamic acid to improve the evenness of skin tone. The pigmentary problems usually tend to settle only when oral tranexamic acid is used for many months. There are a couple of other options for skin pigmentation. First is glutathione and here is a video on it

    My malasma is 70% gone after taking TA 250 mg twice a day for 3 months. Should I continue for another 3 months or stop?

      The treatment is usually continued for longer periods if it is not settled waiting the first three months. There are studies where it has been used longer than 6 months.

      Sometimes the tummy upset settles with time but if it does not, we may need to reduce the dose to one a day or one every other day.

    Bonjour Docteur. J'ai 21 ans et je suis atteinte de melasma dermique sur les joues et sur la moustache. Depuis j'ai tenté de voir avec des dermatologues mais rien n'y fait, ils n'ont même pas pu me diagnostiquer ce problème qui me hante clairement la vie. Peut être parce qu'il manque ici énormément de dermatologues pour peaux noires...
    Ma question est la suivante: pensez-vous que dans mon cas l'acide tranexamique oral puisse marcher? Si oui, pendant combien de temps cela pourra prendre le traitement?
    Et dernière question, est-ce qu'il est possible qu'il ait des conséquences sur la fertilité et autre problème sur la santé?

      Hello, good to hear from you! Tranexamic acid is an option for anyone with melasma irrespective of the colour of their skin. It needs to be used foe about 3 to 6 months. It should not be used during pregnancy. However, it has no effect on fertility long term. Would also suggest the use of tinted sun screens too prevent the pigmentation from worsening.     What laser do you recommend for melasma?

      I am not too sure about which laser is best as it varies according to the skin type. It may be best to see a local dermatologist about it.

      Sir this medicine can i use for contact dermatitis. My doctor is doing continuously q switch laser weekly twice .is it safe to use q switch laser every week.thank you

      If it is the post inflammatory stage of contact eczema it may be helpful. Certain lasers are beneficial, however the lasers could also cause pigmentation in those with dark skin, so it is a bit tricky. A test patch is usually done before treatment is used. I am not sure how frequently lasers need to be sued as I do not do laser treatment myself. @@winifredfernandes9611

      Ideally, no oral tablet is taken during pregnancy due to its potential actions on the baby. Using tinted sunscreens is a cream and is safe to use during pregnancy. This will prevent further darkening of the skin

    i am also suffering melasma. 2 days ago my doctor prescribe me tranexamic acid 250mg twise a day. any side effect for male?

      The side effects are similar to females except, of course, the menstrual changes. Tummy upsets seem to be the most side effect.

    Dr completing 3 months course for melasma if we stop medicine will melasma hit back .or how we will maintain this.

      It is difficult to predict melasma. To prevent it recurring, it may be worthwhile using tinted sun screens so that the facial skin does not darken. Here is a video on it

      There are other over the counter creams that can maintain the colour of the facial skin. The cream mentioned in the middle to end of this video may be helpful.

      It can be continued throughout and does not need to be changed for the periods. The dose is different when it is used for continuous periods. For melasma it is used all the time. @@wajeehamalik4611
