Two movies that you might find extreme are "Eyes Without a Face," a French film from 1960, which gets gruesome at times. It is a very powerful movie. Then there is "Even Dwarfs Started Small" directed by Werner Herzog in 1970. It is about a revolution in an insane asylum. It is one of the most disturbing movies I've seen.
Man, oh, man, I never see people talk about I Saw The Devil anymore! Such a good flick! A little long, though-isn’t it about three hours or so? It’s been a while since I’ve watched it lol. I’m about halfway through the vid, but have to go do something, so I’ll finish this throughout the day lol. Great vid, dude! Keep it up!
Splatterpunk isn't for everyone, but I think it's awfully try-hard. On-the-nose as hell. If you don't like Jack Ketchum, you ain't gonna like much else. So, while it didn't get elbow-deep in the grim viscera of it all, I thought East of Eden was a terribly haunting, powerfully upsetting and ultimately effecting story. Loved The Stand too. Favorite reading I've ever done.

The 2000 definitely was a wild phase for the French cinema. There are also other movies like Inside and Irreversible that just don’t give a shit about limits. 😅
I remember being squeamish at some old zombie movies, mostly George A. Romero stuff but I come to love them growing up and as I get to see a lot of them from other directors, I got more used to them and ended up having some pleasures on watching R rated movies on cinemas alone, I think I like to see them as a delight if needed to be but extreme horror is one I have hard stuff watching like torture and stuff. I ended up at a conclusion that I like gore, a quick visible and explicit gore but I never liked it when the death depicted as a slow and more painful death.
That's not the most disturbing movie ever made. The most disturbing movie ever made is _Grotesque_ (2009) which is a movie from Japan. If you thought A Serbian Film was a tough watch, you won't be able to watch more than 20 mins of Grotesque, trust me.

Yeah, it's like a 2 and a half hour movie. But it's so tense and suspenseful it flies by

I have seen Inside! It was pretty violent but it didn't get to me as much as these others
I remember reading Peaceable Kingdom, and there is certainly a variety of stories. The stories that are disturbing, are VERY disturbing.
The Girl Next Door by Jack Ketchum messed me up more than any other media I've ever consumed.
I'm well aware of that book, and will probably never read it.
