Going by the real life incident you mentioned here, the doctor has documented that the said patient is a full code but the nurse's interaction with the patient indicated otherwise(DNR). Which of the two supersedes the other here? Asking because you never mentioned that the nurse documented such vital information about the patient and if it was well passed on to other nurses caring for the patient as the nurse who obtained such contrary information is not likely to have been on duty when the incident occurred.
