Does it mean all the antenna elements in a column have the same RF chain thereby transmitting the same data?
Which physical antenna to be hooked to which logical antenna port is purely dependent on the person who designs it. It can done in any way he wants. This is what i think

Yes this is the case of 2x2 MIMO and it is possible to use spatial multiplexing, so gNB may send two data different streams from each port.
