Thank you Father for your beautiful explanation...
    Learning experience...
Whenever there is Manual - Guidelines
    It will be focusing on
1- What should be done ( What is necessary)
2- what should not be done ( should be avoided)
Text structure... Doing something serious is a tough/ difficult job/ work
    Than Modal Verbs
Can, could, must , should, may , might , ought to be, are helpful
Mandatory words... Don't say, Be Clear, Necessary to repeat are clue words
    Synonyms
    Distressing - bad/serious
    Disclosure - delivery/ revealing
    Information - news
And top of all that hedging word in question statement is "Avoid" which is synonymous with " Should not do "
Even without going to text this could be easily answered that option C is right ..

The delivery of bad news is difficult. Arrange to talk to the patient in the company of family, preferably away from other patients. In some cultures, it is not common to give difficult, news directly to the patient. We must be aware that the norms and customs of our patients may not match over own. Often we try to soften the delivery of bad news and confusing the matter, or by saying too much and confusing matter or saying too little and leaving people with unanswered questions. Don't say neoplasm if what you mean and what will be understand, is cancer. Be clear, allow people to understand and feel some of the impact of the news, and then allow them to ask questions. It is often necessary to repeat the information to other family members or to the same people the next day.
