The green leaves are any leaves that are not diseased. That could be from trimming plants, greens from root vegetables (if you choose not to eat them) and similar sources. I hope this helps!
You can add soil to the very top and start planting right away as long as the soil layer is deep enough for the roots of your plants. You will need to add more material after time as the bottom layers start to decompose.
