You said “include keywords at the start” - which is not always true. There are some niches; if you try include a high volume keywords on the Title / SEO Title, Google considers this as spam and you might end-up with SERP demotions. Always better to check the SERP and make sure whether the keyword is being used by at least one of your competitor.

Do you have any concrete evidence of KW at the start of the Page Title being the trigger for a demotion?
