    You are giving this lecture strictly along medical guidelines but it doesn't include the biggest causes. The first is due to fear, which declines the immune system if there is a perception of an external threat. And it doesn't include the nocebo effect. If a person perceives damage of some sort happening in the body there is an immune response.
    When there is no real damage then the resulting inflammation causes damage. This was seen very clearly in the early clinical trials on chemotherapy drugs. From between 20% to about 80% of the patients in the control arm lost their hair and had other immune related problems. And that is because those who were desperate to get well chose to believe that they had the real drug.
    These patients in the control arm who believed they had been given the real drug knew that the real drug would damage the roots of their hair and have effects in other areas. So their belief caused inflammation which damaged the roots of their hair so their hair fell out and had other symptoms too same as the patients in the intervention arm. There is no loss of tolerance. It is a deliberate effect by the immune system owing to a belief that there is damage in the area. The immune system moves to clear away the "believed" damaged cells.
