I have experienced that before..it happened when i put grip on the original grip..thought maybe because of the sweat..so now when i buy new racquet straight away remove the oem grip

Pvc tape also can be applied over wooden handle? Is it really needed?

yes i did that too when i removed the original grip..apply the black tape to cover the wooden handle first then put the cushion wrap

Not necessary...you can replace the overgrip only..normally i changed the cushion after 3 to 4 times i changed my grip
