Crazy theory just for funzies: The reason why lovesickness is seen as fatal, is because children aren't conceived through sex in the OP world. People were made sterile during the Void Century and Mother Nature/essence of the Voice of All Things made it so pregnancy/life can be created through will power, and love isn't required. Just genuine want. Children born from genuine love is selfless desire and passes the will power/life force of the parents onto the child(ren) over time, slowly draining them, which is just thought of as a strange sickness in the OP world that takes hold. Only those with exceptional will power survive for a long time, leaving many kids without a parent or parents. Deadbeat parents, unloving parents that have kids for selfish reasons, parents that don't stick around or think about their kids and their future get minimal to no drain of will and life force. Usopp's mother was left alone and couldn't handle the drain, White Beard saw all of his crew as his children and passed his will to them (why he was so sick). Rouge died using her will/life force to slow Ace's birth, keeping him for 20 months to protect him. Roger gave a huge amount of his will/life force into Ace's conception. It slowly took its toll as his "an incurable disease". He succeeded in his goal and he had a child to live on after him.
Just a comment on the writing for Road Start is that island (when it is used as a name suffix) is usually "tou" so it would be "roodo sutaa tou", that, when including the "island" part, would sound mostly exactly like "Road Start" (but the word for island would be part of the name).

I've always thought that the one piece is below reverse mountain. It's a literal "X" and X marks the spot. Also we know there can be things below the red line as fishmen island and Luffy is destined to destroy the red line
