I do believe, in sweden, we got it from the basic dc bionicle comic. Didnt need to buy a special vakama turaga set, i got mine from a local gas station. Actually, thats how i got alot of unique masks like the "kanohi of time". I would suppose its the same in the rest of europe.
I think BrickLink used to list all four variants at one point. After LEGO bought BrickLink a few years back, they've slowly implamented changes, one of them being back in February where a number of parts with distinct design changes were combined. I'm amazed that the infected Hau was not one of those, or that only the two green forehead variants were merged. Perhaps I am misremembering, but I seem to remember there being four, and I couldn't exactly tell the difference on two of them, thinking that it was more likely just a slight variation within a certain tolerance, otherwise there would be thousands of different versions because someone had more stripe than not or more forehead than not.
I bought that book bundle from Scholastic back in the day when I was 12 or so, I can confirm it came bundled in the cardboard sleeve with the books, but it didn't come with the movie DVD ever. Not sure who wanted this info, but there you go.
I have this mask, but I didnt remember how I got it until you mentioned the promotional comic! I must have been around 7 years old when I got it! I dont know which variant though, its sitting in a bin somewhere in my parents house, haha!
Holy moly, i have one of this mask. I must've been very young at the time I got it cause i have no memory of how it came to me. I didnt really like how it looked back then, red and green being two of my least favourite colours. I had no idea it was so rare and unique
I can confirm that these were included with the movie, I got mine (first variant) with the VHS copy (Poland, where the movies were pretty much the only widely available pieces of BIONICLE media)
That Shadow Lord Tahu's body would be perfect for Makuta. Just swap out the Hau Nuva for any version of Kraahkan, though, the original squid-y one would probably look best.
I don’t think anything makes me more nostalgic for G1 Bionicle quite like this mask. Especially since I more closely associate it with Toa Hydros than I do with Tahu. And if you know who that is, congrats! You’re officially old!
Dug through my collection of classic bionicle masks and i still got this one in Type 4, with the brow and scalp area marbled green. Or atleast, very close, with the small variations here and there.

I can confirm, the first issue came with it. It was also included in another issue later on as well.
Must have been the same for germany, i cant think of how i otherwise ever got that mask.

The mask did indeed come packaged with mask of light vhs, I bought 2 copies as a kid to get two different patterns lol
