What should the administrator create first NT Login or UPN? or it can be both? how about the exchange email.

For example, If user want to use their email-id for sign-in and that can be different from their current UPN. (Your domain is mylab.local and in exchange for email you are using @mylab.com format).
Most of time we use this settings with exchange server and in some other environments due to corporate policies or on-premises line-of-business application dependencies.
