Dairy milk has hidden sugars too. Nut based milk like Almond, Cashew or even Coconut milk are healthier options. Add Dates for sweetener. Add Cardamom powder and Cinnamon powder for flavour. Hope this helps.
No sugar, but since milk and fruits both are cold in nature, they are difficult to digest. Specially banana and milk are toxic.

Mam don't you think it's better to use 2/3 spoons of sugar rather than a scoop of ice cream which obviously contains more sugar.
If you use sugar, it's gonna be thin and kinda watery, which nobody wants. This is a MILKSHAKE, not a bunch of blended milk. And also, you're not eating this everyday, please keep that in mind
