What really makes the theme for me is that as you run you hear the sounds of his knives flying past or into you. Added with the fact he's laughing and enjoying every second of it and it makes for a terrifying killer. Monsters don't always need to be hulking mutated brutes. Sometimes a person with no regard for life and finds pleasure in the idea of killing you is more then terrifying enough.
