Ok so my theory is that the horror was because the main character was in between life and death. If not for the doctor on board, he would have died. The girl and her brother _did_ die, and thats why it wasn't your time to get off the train, but it was theirs. Ending 1 is probably failing to hold onto life. Ending 2 is helping them move onto the afterlife
@@MemoriaMatters They mentioned that it happens to people sitting in economy-class. It's about circulation being cut off from how people fit themselves into the cramped seating. I don't know where i'm remembering this from, but i definitely learned about it somewhere like 5ish years ago

People in the cheaper seats on flights were being packed so close together without proper legroom that it was actually causing blood clots and a few people died. Since then there's been more regulation and airlines have to give a certain amount of room for each passenger.
