This isn't a case of the hunter becoming the prey. This is the hunter and the potential prey being equals. The cat is bigger and smarter, but the lemming is a fierce fighter with no desire to run away. Cats just doesn't like food that bites back. Hence why this cat leaves the lemming alone. It may be a rodent, but quite different from whatever rodents the cat has eaten before.

So for a cat, while it would technically indicate disease, it would be a disease inflicted by the parasite the cat is in symbiosis with. Or at least, that's probably what their instincts would tell them. They can't actually know whether a fearless rodent is fearless because of their parasite, or a deadly disease.
The Cat was worried that the Lemming had a hidden suicide vest, would move in close and yell, "OH NO!!" then explode..... I like the fact thet its owners have given it some rock to hurl itself off of.

This is probably a defense against illness. As typically a rodent willing to fight a cat is unwell or sick in some way.
