I personally think “Guest” is based on someone with schizophrenia. It was so bad for her, to the point where she had to stick a knife through her ear and even bleach her eyes. Fast forward to the hospital, she took off her eyepatch. But to her surprise, that thing was still there. So at the end of the day, there will always be an unwanted guest. You just have to live with it.
I think the guest thing is a ghost who haunts her and looks creepy then when she took the eyepatch off in the hospital it turns out the ghost is a real human but the human feels bad for what he did
