Urologist Reacts to GREY'S ANATOMY | Urinary retention (HE PUT IT WHERE?!)

  • Urologist reacts to Grey's Anatomy episode where a patient comes in with urinary retention after getting stung by "bees" after falling asleep with some honey on him.....
    Not a very believable story if you ask me. In the video I discuss how to present patients appropriately, treating residents with respect, the likelihood of bladder rupture or spontaneous bladder perforation, how to repair a bladder and recovery after bladder repair.
    I finish off with explaining masochism and the prevalence and theories of masochism. Is it a psychopathology or just a preference? Why do people prefer pain with sexual intercourse? How common is it?
    Bottom line please don't delay if you have an injury to your genitals. Don't be embarrased. Come see your urologist or go to the emergency room before the injury gets worse!

    Thank you for explaining that being into BDSM does not mean that you are damaged or have any form of psychiatric problem. I am sick and tired of people always equating BDSM with mental issue (most commonly that bdsm-ers are abused as children).
    Yes, there are people who practice BDSM who have psychiatric problems. But the BDSM is not a symptom of that. And it does not mean that all BDSM practitioners have mental issues. There are people with mental issues who have sex. Does that mean that everyone who enjoys sex must have mental issues? no!
    So thanks again! It’s so rare for anyone on RUclips to address this in a respectful and informed manner.

      Thank you for your kind words. This is soooo important. People need to stop being shamed and feeling embarrassed it can ultimately harm them mentally and physically if they delay care.

      Rena Malik, M.D. I absolutely agree.
      Safety is the most important thing in BDSM. But something can always go wrong. Nobody wants that, but it happens. The way people look at BDSM makes it that we sometimes rather “wait it out” than seek the needed care. Having 15 years experience as a Red Cross first aid volunteer I understand the dangers of denying care. Being active in the BDSM scene I myself would never delay getting care if it’s needed. But I do understand why people are scared, embarrassed or both. I hope that someday, preferably soon, that will chance. Videos like your will help to accomplish that.

    • @mrpddnos
      @mrpddnos 4 года назад +4

      JoeysRattata that is precisely why I am so happy with the way Rena addressed it. This way it keeps it vague for those who do not wish to know about bdsm but sends a great and clear message to those it concerns.

      Most kinksters like a little agreed on kink with their pleasure. The ones who really need understanding medical personnel are the ones who enjoy edge play. Things like scarification, needles, rope, suspension etc which all have potential for horrible injuries. We try to be safe, sane, consensual as well as keeping to risk aware consensual kink, but people are not infallible. Mistakes can happen and we need to not be too scared or ashamed to seek help when needed.

    There was a masochist, walking along the roadside. The masochist had walked for a long while, when the masochist saw someone else coming in the opposite direction. As the person got closer, it was obvious that the other person was a sadist. When they met, the masochist greeted the sadist, "hit me!". "No", replied the sadist.

    It may be a weird thing to talk about, but urinary obstruction is a serious problem.
    I was starting to have kidney issues after being partially obstructed for two days from a large mass in my abdomen. After surgery to remove the mass and relieve the pressure, post-op recovery was a breeze compared to the pain of obstruction. I felt like I could do cartwheels even with my fresh incisions. There was nothing like needing to urinate but being unable. I was doubled over in pain and unable to walk, but the embarrassment factor was so strong, even though it was not something that I did to myself.
    My doctors for that first procedure were not the kindest, but it is so great to hear a professional talk about these issues in such an open way. It's doctors like her that made it easier for me to get help earlier when I started having the same issue again.

    :O 2 years ago after my surgery I couldn't pee. I had nose surgery, to open up my nostrils because I broke my nose 3 times prior to that, and I was supposed to lie in intensive care for 24 hours after the surgery, hooked onto IV. And I couldn't pee. They were pumping me up with fluids and I just couldn't pee. It was the most horrible 24 hours I've ever had to go through. And all that after a face surgery. The nurse was yelling at me constantly, because I had to take my IV out to go pee, and when I couldn't and told her I couldn't she would scream at me threatening to put a catheter. And I was begging her to do it just so I could drain my bladder. But she refused. She didn't believe me I couldn't pee and thought I was just a drama queen asking for attention. It took me around 20 hours to be able to pee again. I didn't sleep that night at all, just sat on the toilet crying. The pain you feel when you can't pee is horrible. And it's especially horrible when you're in the intensive care unit completely alone, and aren't even allowed a phone to call your family.

      I don’t know how I would survive something like that. One time when showering a bit of soap went into my urethra, stinging a bit, but nothing to serious. However, it got irritated after urinating 2-3 times and by next day evening I couldn’t urinate because of the excruciating pain in the urethral opening, my urethral sphincter refused to relax anticipating pain when only a couple of drops came into contact with the opening. I really needed to pee after a couple of hours and didn’t know what to do, I was googling symptoms and thinking I would have to go to the ER to get a catheter. Luckily, I found a solution online - basically, if you have an irritated urethral opening, you fill a glass up with warm water and put your penis in it, and since that causes the urine to dilute and not suddenly burn you when you pee, you become able to pee. It was incredible, and it’s a simple solution which may help someone avoid catheterization in that specific case.

Urispaz is an amazing medicine and I now keep it in my med kit just in case. Luckily the regular few drops, though painful, meant I didn't need a catheter.

    Wow! I'd think there'd have to be at least one real medical case like this where the patient lived to tell about it, because I'd think the chance most hospital patients who'd done this as dying from it would be so great there'd have to be at least one patient who'd done this dare-devil stuff and lived, so there'd be a credible and verifiable storyline for a medical drama such as "GA" to air this kind of situation as believable.

    Not sure if you've read much history of science and medicine, but hearing about the really weird idea of bladder explosion reminded me of one of Galen's (130-210AD) super neato vivisections... of I think a pig - where he ties off the ureters to demonstrate the stoppage of fluid to the bladder, and... hopefully I'm remembering this correctly - I believe it describes even the ureters as ballooning but never bursting... which really goes to show how flippin tough our bodies are lol

      That sound pretty cool! I have seen some kidneys have calyceal ruptures with pressure but it's really our bodies way of protecting us - that is not a surgical emergency like a bladder rupture.

    I like how honestly and frankly you talk about sexual topics. I guess it comes with the territory. It is important that people know that injuries during sex (whether by yourself or with others) can happen to anyone and it's nothing to be ashamed of or embarrassed about. Imagine what would happen if people were too afraid to go to the ER because of broken bones or some other injury? Medical staff are there to help you, not to judge you.
    One thing I think a lot of these medical dramas lack is professionalism. I understand why because unprofessional behaviour is more entertaining and dramatic. It's not true to life though. I've seen you and other medical professionals who react to TV series comment about the things portrayed that they would never ever say or do to a patient.
    I'm a first-year (almost second) recreation therapy student (it's a degree here in Alberta Canada) and even we are taught about professionalism and how it's important to act professionally with our patients and clients. We have whole classes about respecting others regardless of sexuality, gender, religion, appearance, ability, race, and so on.

    So... no one's bladder has ever just exploded from pressure without some previous operation or illness? So the urine backs up into the kidneys?

    • @RenaMalikMD
      @RenaMalikMD  4 года назад +5

      extremely rare. I've never seen it. It typically causes increased pressure up to the kidneys and/or you start overflowing urine and leaking.

    I think another possibility for why masochism is appealing is because pain activates a lot of our sympathetic nervous system that heightens awareness and sensitivity. It's perhaps similar to why people love horror films and roller coasters. It makes sense that when you combine that with sex the pleasure would be heightened as well. The other aspects of BDSM tend to be as much psychological as physical, especially those related to power dynamics.

      The best camera to use for vlogs is: the one you have.
      The video was clear, not vertical, had no mismatched audio, wasn't shaky, wasn't the wrong zoom and didn't have any major problems so it's fine. I rather hear the message than worry about the camera type used personally. But you do you boo.
