Trager is a sadistic, mass-murdering psychopath. He doesn't care if people are willing to accept his definition of "help", because his definition of help is "making people believe" by torturing and killing them. I'm confused by what you mean by "mental institutions should take note."
The reason why I find Trager to be the most terrifying boss in The Outlast is that he doesn’t sound scary or insane, but his horrific actions towards Miles and the other variants shows how completely sadistic and unhinged he really is. He has the voice of someone who would be a coworker you'd go have a drink with after work or maybe even a friendly neighbor. Not to mention that he was strategic and calculating lowering Miles into a false sense of security and offering him "safety" away from the variants that were chasing him only just to ambush him and torture him. I even find him scarier than Chris Walker.
The opening of the male ward segment sets him up perfectly. The bloodied writing on the wall showcasing how this sick monster kills his victims. The rows of curtained off beds with "patients" that he has proably lured and caught with the promise of safety in the basement of the asylum, that one Murkoff Company employee strapped to the chair right before the chase with the other variants,who has been tortured so viciously that all he can do is shriek in terror the moment Miles approaches. Not to mention the document next to a mangled corpse where Trager professes that he's still getting the hang of the whole "Doctor Thing", as if he were a child running around playing pretend. His build up is probably third to the likes of Chris Walker and Eddie Gluskin, and I personally feel that he needed more screen time before being killed off, but Trager is one of the most memorable characters in the game for me.
