MEDICATIONS FOR PAIN - Analgesics (Part 2) - ICU Drips

  • In this second lesson on analgesics, we take a look at the different medications that are available to use in the ICU to use as a continuous infusion for our patients pain. It is very important that we control a patients pain in the ICU and these medications are the ones that we frequently use to do just that.
    We start off talk about our 2 groups, the "opioids" and the "other" medications. For the opioids there are 3 medications in this group and it is important to know that these medications are all CNS depressants. We talk about Morpine, Hydromorphone (Dilaudid), and Fentanyl (Sublimaze) which are all used as IV drips, although FentanyI is the most commonly used as it has less hypotensive effect and a shorter duration than the others.
    We finish out the opioids talking about Naloxone (Narcan) as our reversal agent for the opioids, and while normally given IV push, it certainly is not uncommon to see it being infused as a drip.
    Finally we talk about the "other" medications which really is a group composed of just one medication called Ketamine. We had already previously discussed Ketamine in our lesson on sedatives (**see link below**) but we do another quick review of how Ketamine is different from the opioids. Most importantly we don't typically use a ketamine infusion for just pain control.

    Great lecture. Note in Australia it’s common to use low dose ketamine in an iv infusion along side an opioid usually fentanyl or oxicodine and clonodine. The dose for analgesia is 4-8mgs/hr. This is both in the icu and the floor. Common for large surgery and trauma patents.

      Just everything. It's easy to just turn this channel on and listen while I drive. It's easier than opening up Eagans. I always have trouble staying awake when I try to read.

    Theres hope ti stop the pain ! The pain is 24 7. Demerol is a waiting list a pain specialist to spend. My last years not in pain! This is smart to interview mds rns pas mas to treat me i do study medical school !

    I hate fentanyl they gave it to me after ankle surgery then were going to give it to me again but I said no so they gave me a low dose of dilaudid which I liked because it helped me much better
