  • This newest video is a surgical set up for a “Mako Total Knee Replacement.” I was focused on getting the set up done so I wasn’t talking as much during this one so this is more of a demonstration of the surgical set up, sterile technique supplies and instrumentation that we use for our Mako Total Knee Replacement. I hope that the information in this video can be informative for whoever watches. Thank you again for your support and I wish you all success in all your endeavors.
    Sincerely,
    Derek D. Whaley

I admire Scrubs that do Ortho. I really want to learn and scrub a total joint one day but intimidated by the big setup. Great job, and thank you for sharing.

Don't be intimidated! We're just of weirdos that like power tools and metal components. Once you get past the stuff, it's just like any other case. Most of the rep tray instruments aren't even used, different sizes of the same and similar things.
It's been a bit of time since you posted. Have you given Ortho totals a chance?

I've worked in the OR for over 14 years mostly as an aide and recently graduated nursing school and have been functioning as a circulator. I'm learning to scrub and love ortho which is why I found your video and MY GOD. You may be the best tech I've ever seen. You're a sterile ninja. This video was exactly what I was looking for I've learned so much! Thank you so much for your dedication and technique and for this video. Looking forward to your other content. Subscribed!

Just had mine done 10 days ago. Going great. Off the walker and using a cane. Still a little stiff but much better than I thought it would be. The first couple days after we're tough but you can do it. Keep up with your exercises and physical therapy!

I had a Mako TKR 2 weeks ago. It is pretty amazing. I was awake while I got my spinal. Next thing I knew I woke up in the OR from a refreshing nap. Staples came out today. I have 90 ROM about -5 straight. I can walk without a cane or walker for short distances as long as walls are close. I sleep thru the night for 6 hours, and have started to wean myself off of the narcotics. Looking forward to have the other one done in 6 months are so. The pain of walking is very different now then before.

I had a TKR 26 days ago. I took a look around while I was getting on the OR table and there was a whole wall table full of tools, etc, with a guy in a long green coat. Mine was not this type though. In 4 days I was walking without any assistance. I am very thankful mine went so well. It is different for everyone. Interesting to see all this set up! So much work. Mine had inside sutures that went away on their own and then glue over the whole area. I am glad he didn't do staples. Mine was not with robotics either. Thanks for sharing. Very interesting video!!

Thank you for demonstration - it was useful!
Comments for not following standards: no face shield or goggles worn by the scrubbed person, many supply items flipped on a surface already - against AORN/ORNAC standards, initial wearing gloves - touched a package with bare hands and then flipped a paper to the the sterile field, circ. nurse placed a battery in the casing without a scrub nurse. Another gig No-No is a Count - sponges MUST be fully separated while counted - the way it was done on this video isn't acceptable.
20:10 - people are passing in BETWEEN two sterile ring stands - against IPAC standards in OR.
    45 Deg - is still a Hohmann.
Another comment about the setting instruments on the main table. Advisable is to use some towel underneath as instruments may penetrate plastic table cover. Sharps have been placed on the towel - which is totally great.
Prep sticks were placed with their ends out of the Mayo stand - is it for grab for the prepping person? If yes - again, it is against standards as no one is allowed to pull items from the sterile field rather than a scrub person.

Great video! Love doing totals. When I set up, I get annoyed when people just stand and stare at me lol. After the pans are open and they’re on my field I send them to get the patient and I do a quick and quiet set up while the patient is being put under 😅 I am ready to go as soon as they’re prepped 💪🏼
