Usually your employer will provide the self evaluation template. If you have to create your own, I recommend starting with listing your accomplishments, discuss all the projects you’ve handled successfully while working from home, and include how you’ve met your organization’s values, trained new employees. And mention what you aim to achieve for the next year. Hope this helps
