It seems like it makes the already tight space between the grip and the lens even thighter. I gess that is why you have that small lens on?
Not really. It's snug but you can fit large lenses too, for example a 24-70

No. It's pretty snug
In Ulanzi and Falcam's product page, it says its compatible with A7iv? Can anyone confirm this? I would love to get this instead of their native a7iv cage due to the removable side plate and strap holes.

The system yes but you'll need a cage specifically for the a7siii from Falcam. Not sure if this one would fit it
