The dude tried to sue eminem & filed a defamation lawsuit against him when this song came out & tried to say he made the whole thing up. I think the principal is likely an exaggeration, but the rest pretty much all happened to him at school. D'Angelo tried to sue him over this song, but as he admitted it really happened, so the judge threw it out. His mom actually went looking for him and found him unconscious and bleeding on the bathroom floor at school. She sued the school over it. He did get brain damage from it
