If a mother left her minor child at the ER, they would most likely call Child Protective Services, turn the kid over and she would be facing a crap-ton of paperwork even if she did't lose custody. Especially if the kid needed a psych eval, like the Fire-starter.
Fire needs oxygen. More on that in a moment but keep that sentence in mind. One closes the door to contain the fire. Keep in mind that the fire was inside a hospital so there is patients throughout the facility, therefore slowing the potential spread of a fire in a building full of people is usually a good idea. Fire needs oxygen to breathe. Denying the fire that luxury is a good thing. That's why one never opens up a door that feels hot or warm to the touch, hence a backdraft which happens when more oxygen is introduced to the fire. Stairwells in schools are isolated with fire doors for the same reason, to keep the fire contained and out of the escape area by providing oxygen to those who need it, to escape safely. Doors are often a good thing in these cases because it will keep a lot of the smoke out of the room for some time, which also helps in preserving needed oxygen for anyone who may be trapped because inhaling smoke can kill a person. I'm sure you're familiar with the term "smoke inhalation" Any medical professional should know this in an event of a fire within a medical facility such as an ER. So, there's your answer. For the reasons you described, this is an incredibly reckless thing to do and could cause him to pass out before being able to extinguish the fire. Shutting the door would be a smart thing to do if you had no means of extinguishing it and nobody was in there, but here it doesn’t make sense
