There will be no additional license required for Adaptive Milling. The Bottom Up option will also be included. Currently, it is under Preview and not in Production in NX12.0.2 I misspoke in the video, my apologies. The target date is for the release in January of 2019.

These environment variables are not public yet. Once all testing is complete by Siemens, it will be available in the new NX release. The new release is slated for January 2019.
