They are responsible for the good setup and maintenance of their machines. Any malfunction is a failure to properly manage the equipment. The only valid reason for the casino not to pay is to have proof of cheating.
The casino should be forced to pay AND face a fine for bad maintenance plus a review of the incident by the gaming committee to decide if they can keep their licence. Then they can support the cost or seek compensation from the manufacturer depending on the real cause of the problem.
If bad maintenance is considered rightful cause to not pay, this means the casino has an incentive NOT TO maintain the slot machines so they have an opportunity to void any big win whenever they want.
As long as the player is concerned the contract with the casino at slot machines is "you put money in, you don't cheat and you get the amount displayed on the screen". If the casino says the machine is malfunctioning and refuses to pay then they must cancel all previous plays on this machine and reimburse every single coin that was put in it.

When people are actually playing on these machine, malfunction voids all pays is at default. Meaning pretty much the machine don't pay at all. Patrons are losing 99 percent of the time. Anyway compensate her with something like $20,000 instead.

Is there really any such thing as a penny machine? Most of the penny machines say 1 cent on them but then have a minimum bet of 40, 50 or 75 cents and maximum bets of 4 , 5 dollars or more. The machine clearly said she won a million dollars so the casinos should honor what the machine said.
