    I have never placed a regular sized implant--just placement of a restoration afterwards, but I am always curious as to what the periodontist have to go through. By watching these videos, it gives me ideas as to what to request for and not ask for the moon! Thank you for all your hard work in putting all these videos together.

    Dr. Simon, you spoke about the blood vessel, if it is severed, and bleeding a lot, what specific protocol would you follow, any changes or just carry on with grafting and close the area..? Thank you.

    What is the best bone graft. Can you do this technique without bio membrane.

    Very useful video .In Australia this kit is not available when you log in and search shopping-instrumentation you will find only this kit ( Hammerless Crestal Sinus Lift )

    Can you comment on how you deal with septations in the sinus.

      That depends where they are. Sometimes by creating two windows. And making the window very large. Total compartments are not easy to handle. Sorry I don't have magic. When I see a septa on the scan I take into account a perforation and need for management.

    If you care about your patient's then you will send them to specialists that have actual residency training. I condemn anyone for thinking they can perform at the level of a specialist without adequate training. If you need to watch a you tube video to learn a procedure then you are thinking unethically. Be careful and always remember lawyers, patients, dental/medical boards and specialists are unforgiving if you do harm.

      Pressure, local anesthesia and gauze soaked in lidocaine and epi 1:50k

    Great lecture, I would like to ask a question; Is it possible to have a permanent headache after implantation of edentulous patient if the bone is thin and weak? Thanks for your answer

    Hi Dr. Ziv,
    In the presentation you mentioned about the blood vessel passing through the lateral window, I would like to know that how should one avoid any damage or nick to the vessel if its passing within your window outline ? Also if you could suggest how to handle the vessel bleeding in case it's severed.

      Thank you for the feedback. I came up with this complexity level determination some time ago. It's described in another video that demonstrates treatment of a lesion in the esthetic zone.

    hi Ziv, very usefull considerations about sinus lift but what about the vertical deficiency of the bony crest?do you think it 'd be better to perform a vertical gbr simultaneously or to correct it prosthetically maybe with some pink porcelain? thanks. sandro

      alessandro zussino It's possible to augment vertically. However, not easy to do and also not always predictable in this region. So unless the vertical deficiency is not severe it's better to compromise on a longer crown.

      alessandro zussino you could get a custom made 3D printed titanium mesh, along with autologous or allograft material with BMP-1 and create the vertical bone needed for an ideal prosthetic outcome or refer to a surgeon who is completing that type of technique. Dr Pikos in Florida is one provider that comes to mind...

    What if the vessel was bleeding?shall I ligate it? And if the membrane was perforated?

    Why you didn't skipped the window approach and just drilled for 2 mm with a round stone and a lot of irrigation and use the sinus lift instrument and add membrane and bone grafts and wait for 3 month for better and secured placement and avoid dislocations, avoid also bone resorption from reflection of the flap done for the window.

      Asma Ayoub it's an option but think it'll be more predictable with open approach.

    Thank you Ziv for this clear presentation. What is your attitude regarding the bony window wall when you use the piezzo , do you raise it up with the Shneiderian membrane and use it as roof of the sinus or you will replace it to close the lateral access ? Thanks again. Elie

      Thank you. I do use if as a roof and don't detach it. The membrane is more inductive for bone formation but in the big picture it doesn't matter in my opinion. Quite often the bony window comes out by itself and then i usually discard of it. There is an element of risk to remove the bone in terms of a perforation if there is an adhesion
