And he's mentioned not being alive multiple times in other songs because he knew the drugs were gonna end him because if he stops taking them, then his depression and anxiety will make him want to kill himself. So the pills were his "motivation" to live and would be the result of his death
I think the reason he said “you can see the pain in my laugh” is because his laughter which appears to be happiness is contradictory to how he’s feeling physically, and physically he’s broken because of the relapses and drugs he’s taking, and the laugh is cover up.
