What do I do If the power going to the mosfets are 120v dc, and the mosfet gate drivers are using separate 12vdc. How can Vs on the gate driver connect to that high voltage on the mosfet shouldn't they not be connect and stay isolated what connect to Vs on the mosfet driver then beside the bootstrap circuit capacitor from VB?
We are driving two MOSFET here, the one with floating source terminal is driven using high-side while the other having source terminal connected to ground is driven with Low-side drive. If you have one MOSFET to drive you can use either the high or Low-side driver depending on the source terminal connectivity.

C2 must hold enough charge to turn-on high side MOSFET. It depends on duty of high side mosfet

Yes! You can need three driver ICs in that case.

You can leave one side floating. Either use high or Low-side driver is fine.
