If i get a nickel everytime i've seen Death not appearing when she should have, i'd have two nickels. Which isn't a lot but it's weird that it happened twice. In all seriousness, seems like Nier is kinda buggy lol.
The SBA appears with Death is. Since he left her across the screen, she stayed there when he did the SBA. You can tell if she hasn't returned yet if Nier's normals are the weaker versions she only does when Death is deployed. Using a skill that repositions Death in front of you before using an SBA fixes that situation.
