    It’s my belief that the anesthesiologist is the most important doctor in the operating room! I wouldn’t have any procedure or surgery without an anesthesiologist there! How foolish to not have one there!

      They will run the code besides managing airway if patient gets into trouble so yes they are the most important person in the room during this situation. Everyone is important in the surgical room and has a role with purpose. This should never happen on his watch ! I hope these cases are discussed in fellows and residency programs. What not to do.

    As a medical assistant i would have called 911 when he said do not. F#@! a job. That persons life was on the line.

    My late husband was a plastic surgeon and he would NEVER operate on a patient in our
    private OR without a board certified anesthesiologist. I was the practice manager.

      No one who understands how a plastic surgery office works believes you! Many procedures are done in office. You aren't going to have an anesthesiologist in this situation.

    I almost did this exact thing a few years ago. In the office with no anesthesiologist. My husband put his foot down and said absolutely not! I was kinda mad at the time but now I’m really really glad he intervened!

      I believe I experienced this to some degree receiving liposuction while awake by a cosmetic doctor. I remember feeling like I was going into a dream state (I was given no oral medication), felt anxious and my legs began twitching involuntarily and the nurse had to hold my legs down, ensuring me it was normal. After the procedure I was left to change and began sweating and feeling like I was going to pass out. Ended up having to lay down with an ice pack for a bit but was okay- they advertised that you could go back to work same day and drive home. I ended up calling someone to get me because I was in no shape to be driving. I remember thinking something was wrong and worrying about how long it would take for an ambulance. Super scary. Wish I understood the risks more then. Never would have done it and would never do it again

    I have local anesthetics injected in my neck and shoulder for pain control. During this process through the twenty some years i had the unfortunate luck of being in a very secluded area. The doctors rotated and came in every six weeks. The one that did my injections went overboard. I actually stopped him from doing more. I was burping the lidocaine and felt horrible. This vid rattled me. I am so glad I stopped him.

      I gain some weight last year, because i have had broken my leg, it goes not as i expected, but i know, i have to have patiency with the proces of loosing weight naturally. Liposuction isn't solution for me.

    When I had my last child, the anesthesiologist hit a blood vessel with the epidural. If he hadn’t acted quickly, my daughter and I would have both died.

      Epidural goes into spinal cord. There are vessels there. Basically so it won't happen the needle has a bit different shape.

      Oh mygoodness.
      In had an adverse reaction during my epidural. I loss consciousness and was rushed into surgry for a c section. The scarest moment ever because i had just come back only for them to put me right back out because the epidural hadn't started working yet.
      I just asked the lord to wake me up. Me and my son are here now alive and well.

    WOW!!! I am a transplant patient who just did a consult for liposuction. I am So Glad I have come across this video. You just sealed my decision of NO, I am not risking my second chance at life by trusting anyone won't kill me in this procedure. I already had a bad gut feeling to not do this. Ill keep my little bit of flab and Live!!

    That's pretty scary. I live just outside of Pensacola, FL. All my doctors are in Pensacola. I probably wouldn't have seen this if I hadn't seen your live. I'll def be following the case. He must have totally panicked In that 20 min he waited. I can only imagine what thoughts were going through his head. He was surely very overly confident of his skills.

    3. I had a condition that started years ago and lasted about 6 years that made me need to use prescription lidocaine patches over half my face and then down my neck and shoulders. I couldn’t understand why my doctor limited my amount of monthly patches to a measly 200 (just a guess) a month. It didn’t seem fair with as much pain as I was in.
    He tried to explain what you just explained, but he didn’t do a very good job (or I was in too much pain to get it).

    Great live stream man , this was a very sad story, it’s always important to have an anesthesiologist or CRNA present at all times! If I was an anesthesiologist, I would be a great anesthesiologist like you! I really enjoy donating to the anesthesiologist foundation since it was hard for me to go to college because of my cerebral palsy, I really wanted to be a anesthesiologist, it takes a smart and special person and skills to be a anesthesiologist or CRNA and you’re one of them! I look up to all anesthesiologist and CRNA! You rock

    At least 20 years ago, We had a young lady land in our NC ER. She was going for laser hair removal, and the doctor at the laser clinic provided cream with lidocaine for her to apply every where she was going to get the laser treatment. She started feeling unwell while driving to the appointment and was taken to our ER. She coded, and never recovered. It really shook up the doctors working on her. Young, vital, dead. Tragic.

      We had a case of topical toxicity - woman used lidocaine gel before using mechanical depilator and, either before or after, went tanning. Fortunately she recovered but with minor neurological complications, if her daughter didn't find her...
      Similarly, there was a case of lethal topical toxicity with a female patient who used lidocaine around her pubis using film occlusion and heat, likely crossing mucosal barrier into labia, before laser hair removal. It was thought impossible to overdose this way, with such a small contact area, no matter the gel concentration. Incredibly tragic that it was found this way that it is indeed possible.

    I have strange reactions to anesthesia. I had my gallbladder out in 2018, and they almost stopped the surgery because my heart rate and bp went through the roof. I woke up so very sick, and I was vomiting, had a massive headache, and was shaking uncontrollably. I felt like I was dying. They ran some tests on me and I was in the hospital for an entire week, but they couldn’t figure out what had happened. They said my body apparently reacted very poorly to the anesthetics, and the anesthesiologist had to give me some type of medication the bring my bp down that they keep on hand but don’t use all that often. I cannot remember what it was, though. A couple of years later, my neurologist had to take two nerve biopsies on my leg. She gave me the lidocaine with epinephrine, and the same thing happened, just not to that bad of a degree… my heart rate and bp were really high, so I was shipped off to the ER. They just had me sitting in a room. They ran no tests on me other than an ekg, then a couple of repeat ekgs, then eventually discharged me. Finally, a couple of months ago, I had to see the dermatologist to have a suspicious lesion cut off my cheek. They loaded me up with the lidocaine/epinephrine, and within about 30 seconds to a minute of the first few injections, my heart started racing. Then I started shaking horribly. All of the color drained out of my face, and I asked them to take my bp. It was stupid high, and so they called the dermatologist in and she was talking to me. I felt like I was having a panic attack, so they covered me with blankets, turned off the lights, and allowed me to rest for a few. I carry zanaflex around with me, as I have bp that can get reallllly high and doesn’t respond to typical bp meds. She had me take 3 of them. I started feeling a little bit better, so she decided to try to go ahead and cut the lesion out. They let me go home to rest. About a week later, the SAME THING happened again… this time though, I was at home and hadn’t had any lidocaine and epinephrine… but my heart was racing so fast and my bp was super high- I felt like my chest was going to explode, I began to POUR sweat, and I felt like I was really going to die this time. I got to the ER and they ran a bunch of tests, but found nothing… again. 🙄 They admitted me and gave me some IV anti anxiety meds, which did help. The next morning, my internist came to see me, and he ordered some tests, and lo and behold, I have either a pheochromocytoma or a paraganglioma somewhere in my body. My bloodwork for metanepherines came back showing an ungodly amount of norepinepherine, so now I have to have some scans so they can find the little bugger and take it out. 🙄 I’m so lucky to have him! He came to see me and knew EXACTLY what was wrong.

      A good thing to note. My BP can suddenly jump but is usually very low. And anesthesia has given me that exact reaction. 😮

      I was barely thru your history when I knew your diagnosis! That’s classic basic med school 101 for your reaction! One of my buddies in med school diagnosed a patient with pheochromocytoma!! Somebody should have diagnosed you waaaaay earlier!

      100% agree. From what I read he was doing several things on her at once and with “type “ of procedures he was doing can they be done under local? This will be interesting to know - was there a consent form obtained before the surgery ? Is the surgery he provided typically performed with anesthesiologist vs getting a local ? did the surgical team all have bls acls training and was this up to date ? Was there a crash cart available in his office suite ? Was the office trained on what to do who checks the medication cart for expiration dates ? Did the office typically use anesthesiologist? If he hired unlicensed personal he takes on full responsibility of what happens. It’s up to him to do the right thing. He would need to show he’s ACLS /BLS trained. How many codes did he run ? Was he up to date ? I would never ever want an office like this working on me or my loved ones without anesthesia available to assist with airway. It’s not like this guy was removing a simple mole here! Will be interesting to see how this ends.seems to me the regulations are not verified as well as they should be in office based surgery centers

      We have limits. So no more than X ml of Y mg/ml.
      Very small amounts are used for stitching tiny wounds. So anything bigger than the small amount you use for a tiny cut, is an anaesthetist or Just Don’t. Anaesthetists can use nerve blocks instead. Small amount in nerve stops pain.
      Was this even a doctor?

    Reinforces to me as an RN to never have plastic surgery that’s not medically necessary. I seem to have a good self esteem and accept myself the way I am.

    I’m a dentist. I’ve definitely heard of lidocaine toxicity, especially when doing multiple extractions. Sedation (IV) is helpful in that it decreases the need for supplemental lidocaine. Also, using Marcaine for long procedures helps in that in provides longer anesthesia thus decreasing the need for supplemental anesthesia.

      How much does this happen in dentistry? I'm one of those people that needs at least twice the normal amount to even come close to numbing anything and usually end up numb from my mouth to my eyelid.

      Registered Dental Assistant for 23 years. So glad that we never experienced a tragedy like this. I prepared the anesthesia for all procedures. Thank God for Carbocaine. Nice to hear from a Dentist.

      Have u ever had a person with double vision after getting local anesthetic? I’ve had that one time before and I felt really light headed too, went away after some hours but it was really unpleasent

      @@aedenpik the lightheadedness, at least, might be from epinephrine mixed in to help the effectiveness. I have to specifically ask for plain local anesthetic for dental stuff or I feel like I'm having a panic attack.

    I never go thru with a procedure at a surgery center without making sure that there is a competent, licensed anesthesiologist there. It is a highly complex field that most people don’t seem to recognize as vitally important.

      It's a best practice. It's rare it's forbidden because you may be the only person available in an emergency, but the rule of thumb is that you never operate on family.

    I have a pain pump . It is loaded with Marcaine
    I had injured 3 vertebrae, not operable and my pain was outrageous. After an ugly dance with Dilaudid and morphine, they tried the Marcaine. It helps, but not as intense pain relievers. I am a doctor's nightmare! However, Something is better than nothin̈g. The pump is filled once a month. I never thought that Marcaine could be filled in a pump. Dr. Kaveh, You are the best! Thank you for sharing your education with us. It truly makes a difference! You make this world a better place and I thank you! ☮️❤️💯🌝🐕😺

    It is against the law for a family member who is a doctor to preform any medical procedure on a family member and in this case causing the death of that family member! So the former Dr. Brown deserves to go to prison for life! Px

    irs so scary, many years ago I had a procedure done, It HURT, thankfully I was in a hospital, the anesthesiologist said “she can feel this”!
    He was right, I was in pain but was paralyzed and couldn’t let anyone know

      Vital signs probably. Usually, when patients can't talk or move we associate a rise in heart rate and other vitals with pain response.

    I would like to know why he was allowed to use Lidocaine. FDA has been issuing warnings on it since 2009. My Dr. In Pensacola quit using for biopsies in 2009. Is also like to know how this man has been able to keep practicing with complaints since at least 2019.

      Any drug is poisonous if it is given in inappropriate amounts. Including lidocaine. It has been used for decades every day safely by doctors, nurses and dentists. Because it has been so safe people using it get a false sense of security and forget it has an upper limit on dosage.

    I've had some large lacerations repaired in the ED and so i've gotten the high end of max dose/kg of lidocaine a couple times. One time they used both lidocaine and bupivicaine to cover a larger area. Can't imagine having such high doses of local outside of a proper OR or ED setting!

    I once had a paradoxical reaction to Versed for a breast biopsy. Now whenever I need a procedure I warm them repeatedly of my reaction to versed. The surgeon didn’t recognize my reaction until I called him later from home and told him. He was a general surgeon. I never went back to him!
    Strangely, my son has experienced the same reaction to versed during his gallbladder surgery. Inherited allergy perhaps?

    I'm an NNP (neonatal nurse practitioner) two things, about midaz and GA& pregnant women: If a baby comes out anesthetized, it is imperative that there be a trained neonatal resuscitation team present and ready to support the baby. When mothers are given C-sections under general, they work as quickly as possible to minimize the exposure of the baby to the anesthesia, but that is not always possible and frequently if there is a prolonged exposure, those babies need a breathing tube for a period of time until all the anesthetics wear off. And re: midaz, it and other benzos have been shown to cause neuronal apoptosis in neonates, though it's a little weak on evidence (flaws in studies and largely on animals). We try and avoid it when we can.

    It happened in my country few years back , whereby a prominent politician son in law was given sedation during a dental procedures but developed cardiac arrest at a private dental clinic with poorly equipped emergency devices. He was pronounce dead after a futile resuscitation at a different hospital.
    The sedation was given by a gynaecologist 😮

    I had a friend once who confessed to me that as a 13 yr old she was using drugs and alcohol. She was very confused and her father was an anesthesiologist. She went through his bag and found something and wanted to self cancel so she injected it into her arm. But it was just a local. We always laughed about it. How she had to eat dinner and act normal while her arm was numb. It was sad too because she wanted to end things. Anyway, now 25 yrs later I realize she could have had her wish that day. I'm very glad she was not successful. Who knows what she injected.

    I had a Nuse Practitioner, for my Suboxone Program, I got to know her pretty well, She was telling me She had a Boyfriend out of the country, She was going to see him soon and wanted to look good, She told me She was going to have Lipo Suction done, I saw her one last time before she was going to get her Procedure done, I found out She died on the Table, I was so upset, She just wanted to be more Beautiful, She already was, and She was killed getting Plastic Surgery😢😢😢😢

      I am a nurse practitioner and years ago, one of my co-workers, who was a beautiful NP, married to the love of her life, wanted to lose weight and have a baby so she went in for gastric bypass and passed away a week later after bleeding out from a "knicked" liver they didn't pick up on. When they took her back to the OR for emergency surgery, she aspirated and became septic. She was in her early 30's and brilliant. RIP Robin❤

    Cutting corners to save cost is more prevalent in private practice. I have worked for 33 years in a university ambulatory surgery center and saw how liposuction practice has evolved among our plastic surgeon. Lidocaine toxicity is always a risk but that is only when a large anatomic area is being liposuctioned.Also the anesthesiologist is made aware how much in injected and the dosage .

    Listening to this is making me think of Joan Rivers. Question: Based on what you know what procedures would you NOT do outside of a hospital setting? Example-dentists regularly use lidocane. Oral surgeons often do thier work in their own clinics. I'm interested in your opinion. THANKS!

      Great question. I've wondered about that, too. Colonscopies are done at my gastroenterologist's practice with an anesthesiologist or at a hospital; they leave it up to the patient to decide.

    Here in New Zealand no surgeons will ever do liposuction without a general anaesthetic, there must be some very strict rules that they have to follow around this. They wont even use a local anaesthetic for a lower blepharoplasty. Plastic surgery is very expensive here.

    I would argue that the man’s physical appearance has absolutely no bearing on this matter. His wife just died. He probably not looking his best. I would also argue that a doctor is a doctor once they complete their level of education. The man is still a doctor just not practicing.I love the information you provide and delivery. I would be cautious to overly criticize colleagues even when they are clearly an error. I think the main problem was performing a potentially dangerous procedure on any family member. Lose perspective and panic. That’s never good in a resuscitation.

      No. This person did gross negligence! He waited 20 minutes to call 911! If you cared about your patient you would call immediately when they start having a seizure! This guy is indefensible. And if you think he's not it's probably because you're doing or want to do the same

    Dr. Ka v I have a question (lol I have so many questions) I wonder if you would be willing to discuss your personal opinion on and /or medical opinion of alternative medicines such as Rock Tape which uses the principle of kinesiology, My story is heartbreaking I was injured in service so the way the damage is ...is...is not seen often in non-military practices (many of us denied the care we need due to us being National Guard & being released back to the state when returning severely injured (ts a dirty secret they don't want out so we are once again silenced) further devastating us adding to the emotional, trauma. Also, treatment cannot be typical or in that box, sadly what is currently happening to me is bad, yet it is typical, fighting for medical care
    I'm really interested in your work with PTSD BTW many of us are too afraid to mention anything

    I got lidocaine once while getting a steroid injection. When leaving to go home, my throat and tongue went numb. I turned around to go back but by the time I got back it all went away. I now say I’m allergic to it, but other Dr.’s sometimes try to say it was probably something else! I feel it was the lidocaine, so I don’t want to take any chances. The question is, can you be tested to really find out? Thank you! 😊

    When I had a operation which was using local anaesthetic I had a anesthesiologist check the type of anaesthetic drug against a chart for the procedure that they would be doing on me and my hight weight wait and medical history for any previous problems with serten types of anaesthetic drug also what medication I was taking to check that it wouldn't have any bad side effects on my heart or my brain due to me having cerebral palsy and epilepsy. She then then gave the surgeon the syringes inject into the aira that needed to be operated on he gave me 1 injection in small amounts and in different parts of my arm and totally dead I couldn't feel a thing

    Watch his video on his personal channel where he introduces himself and brags about his accomplishments. Watch his blink rate and look at his eyes. I firmly believe he’s under the influence. I believe he’s been operating under the influence of various narcotic pills. That’s why he went from total success that he described to making reckless decisions for several years now. Pill addiction- my theory

    There has been reported cases of TOPICAL lidocaine toxicity, there have been multiple cases of "safe, awake plastics procedures" going awry due to lidocaine toxicity and this man, a board certified doctor, operating on HIS WIFE, was this reckless?
    I'm sorry but if there's no documented history of negligence and missing training this sounds like premeditated murder.

    Once had a dentist hit the vein during a filling. He immediately placed direct pressure on the area. I just had swelling in the cheek but no other reaction. The dentist had a great scare though.

    Dr. I take diltiazem for SVT. The last 2 dental visits I was injected with bupivicaine with Epi. I had several minutes of rapid , irregular heart rate. I'm wondering if I should avoid the epinephrine?

      I thought it was suspicious right off, and every extra detail further supports it being a deliberately staged "accident." I won't be surprised in the slightest if this case shows up on Forensic Files in a few years.

    The medical assistance should be charged, too. They should have called 911 even if the doctor refused to. They knew what was happening and knew or should have known the risk of her dying; they have a duty to the patient, too.

    I have EDS, lidocaine doesnt work very well for me. I reached the maximum limit during a dental. Appt, for a filling. Doctors still want to use it on me. I will add “no lidocaine”
    to the consent page before signing.

      Also have E.D.S. and same problem here! Often, it just doesn't take at all or wears off extremely quickly, while the procedure is still underway or immediately after. 🤦🏻‍♀️🙈

      Me too--furst I have heard of this, so scared if dentist always asked them to load me up with novacaine...
      and I have lidocaine skin patches for a bad wrist. And have nonchalantly used them--i am really chillin on them now hearing this--i knew early on to peel them off if heart got jumpy after few hours, didn't know it was that somewhat fNgerous. I always cut them in half but previous to hearing this complicated stuff, I thought they were mild and harmless.

      Right! Imagine stitching yourself! Being in g to he Medical field, I believe I could do stitches on someone else, but not on myself-that doesn’t seem normal though, that she thought that it would be normal for her to stitch herself up. He must be a real piece of work. What an IDIOT.

    This doctor performs a 'rare' or very specialized surgery, that I came very close to seeing him for - a DIEP Flap Transplant. WHEW! Dodged a bullet there! When I watched a video by Christina Randall, I recalled things that you'd said about lidocaine. So thankful for the knowledge you share!

    This sounds like murder. He continued to give her lidocane even after she started showing signs of toxicity. My dentist and dermatologist has explained this phenomenon to me. Im not a doctor and im aware of this. The doctor in question knew better regardless of him not being trained as an anesthesiologist. Sickening. I feel bad for the girl aasisting in this evil.

    I have to put 5% Lidocaine patches on my back because of lower back surgery. I also have fibromyalgia. I put the 5% Lidocaine patches on other parts of my body. Is there a certain amount of Lidocaine patches that I should not put on my body per day???

    During a steroid hip injection procedure done under a local, instead of the lite sedation I was suppose to have, my anesthesiologist was a nurse. I thought anesthesiologist were all doctors.

      Certified Registered Nurse Anesthetist?
      You would be surprised at how often it’s a CRNA. My emergency surgery was with a CRNA instead of an anesthesiologist
      Then there are sedation nurses that do stuff like moderate sedation for colonoscopies.

    All cosmetic surgery is risky and the person undergoing it says some good prayers before undergoing it. Idk why the husband would take such risks but his wife believed in him it seems.
    I imagine the assistant is very traumatized.

    I once had a significant amount of Lidocaine enter my bloodstream from a deep injection in the dentist's chair. It sent my heart racing badly and I would never want to experience it again. Not sure how often this can happen and whether dentists prepared for it. I would also be concerned with oral surgery since anesthesiologist not usually present.
