I've encountered the issue you describe before, but I've actually never been able to know what causes callouts never to come. I'll usually leave it on until an actual callout finally comes (can take 10-15 minutes), then save the game right at the start.
I didn't post the file directly, as people may have varying results. I basically disabled all other callouts and when the endless freeplay started, waited for this one to pop up. Then saved, as it can be a bit finicky. The longer you fast forward, the more people will be attracted to the center of the callout. To keep it peaceful, you have to isolate the guy with the picket sign, and use overwhelming force of a good dozen cops to quickly arrest him without turning the protesters agro. If you succeed, you can then easily let the cool down time run out, and just have officers redirect people off the street. Unfortunately, that's where it ends. If the crowd goes agro though, all of your first responders will die. lol I wish I knew how to give them super health, and I wish not all the protesters go agro, but ey. But in the freeplay parameters, all you have to do is change the enabled value for every callout to zero, except for EFPDemonstration. For that callout, just need to replace it with this:
