- Clinical examination of the lymph nodes includes inspection and palpation of all clinically relevant major lymph node groups. Clinically relevant lymph nodes are located in the head and neck, the axillae, and the inguinal area. Palpable lymph nodes should be assessed for size, tenderness, mobility, and erythema. A complete examination of the lymph nodes also includes the palpation of the spleen, which is considered a major lymphatic organ.

Nice and concise. Axillary nodes can be conveniently palpated with the patient's arm resting on the examiner's forearm.
I think the reason they initially did it with the hand raised was just to show where exactly to palpate. And they clearly stated it should be done with the arm relaxed.
Virchow node is super rare... and entire university they are pushing this idea... but when we had oncology in 5th year... oncologist told us that he saw it only once in his 35y carrier
