3-4 sounds good for most reports. It depends on how much information is needed though.

Hey sir hope you're able to see this before my exams. For all reports to we have to use the same format or can we leave out the addresses and date and just get right into it starting with ''dear _____'' ?

This is the standard format, but pay attention to the question and let that guide you. There are different types of reports, so there are some variations to the format.
