How can you get the service to not auto-end after a train has crashed?

When you see the message that says "Service Failure: Vehicle Derailed" or something along the lines of that you have to go into the pause menu and quickly save the game. Then return to the main menu and load the save file

How on Earth did the first and last videos happen? Surely if there’s a train already on the line ahead it would show a Red signal which if you pass would immediately end the game and take you to a menu asking you if you want to try again from the very start?
