I was diagnosed with 7 polyps on the gallbladder, the largest of them was 6.5 mm (10 months earlier it measured 5.5 mm), the surgeon ordered to cut it out, and so it happened. The gallbladder was removed and in fact it turned out that there were polyps in the gallbladder (the doctor assumed it was probably gallstones and not polyps), but it turned out otherwise. The gallbladder has been sent for histopathology, what are the odds of cancer with that many polyps? largest 6.5mm the rest about 3-4mm. I'm 29 years old.

This is a very well presented explanation of exactly what I am experiencing..currently have a gallbladder polyp that is 8mm, in Jan 2020 it was 7 mm..no gallstones at one ultrasound and 1 gallstone at another ultrasound. I do not want to jump in and have my gallbladder removed as the consequences that I have read about can cause extreme dietary distress. I am currently asking doctor for Ultrasound every six months to monitor growth and from listening to your presentation I am comfortable at setting a limit of 12 to 14 mm before I agree to surgery...Does this seem reasonable? I have no symptoms and am not of Indian ethnicity and am 74 years old. Could I wait for 1cm in size before surgery and still be relatively safe for cancer spread?
Thanks for your comment, I am unable to provide individual advice. However, the presence of gallstones increases the risk of malignant change within polyps. Hence the recommendation. The internet platforms are replete with misinformation around the consequences of gallbladder removal.
Look into homeopathy. In particular, Indian homeopathic dr’s. It is very VERY possible to shrink gallbladder polyps, can be EXTREMELY difficult for some but it is absolutely not impossible. Many scientific studies have been uploaded online which show a few things that have been used to shrink gallbladder polyps. Type in on Google “ shrink gallbladder polyp report” or more specifically type in “ Carcinosin 200 gallbladder polyp shrink report” or “ Lycopodium 200 gallbladder polyp shrink report”. These reports have been uploaded by no bias scientific institutions. There is some very interesting info in them. After reading, find a good homeopathic dr, preferably Indian as they come across this particular problem a lot. Again, it can be very difficult for some individuals to shrink their polyp compared to others but it certainly is not impossible. It can be done.

I have been doing the ultra sound every 6-12 months to keep a check on them. The biggest polyps is around 9 mm and it has been like this for last few years. Can you please suggest if I continue to monitor it or have the gall bladder removed despite the fact that polyps have not grown in size in the last few years. But the problem is i just started keto diet 2 days ago (blood test shows my liver is a bit high (bili total) but the liver ultrasound result was good. When i look it up the cholesterol polyps might be cause by too high cholesterol. I dont know if i should continue doing keto, i only plan to do keto for 2 months after all.

I'm glad to have find this channel. In Dec 2019,. because of a horrible pain I've experienced, it was determined via scan that I have a polyp on my bladder. The pain was unbelievable. Long story short, I still have mild pain from time to time, nothing severe. I was warned not to do heavy lifting, stay away from diary products etc. At least not to over indulge. I don't want my bladder to be remove in the long run. Any advice Sir, concerning a diet that is good for someone in my case? Thanking you in advance.
In general, polyps 6mm or smaller in size turn out to be cholesterol deposits (which are not true polyps). As mentioned in the video, if suspected polyps are associated with typical biliary colic (pain from gallbladder), removal of gallbladder is advisable. Please seek and follow the advice of your healthcare provider.
@@thesurgicalscholar what about all the issues caused by removal? How do people absorb fats? How are waste products removed from the body ? If the bile is constantly dripping, there will be non left by the time you need it for digestion. And it won’t be concentrated. So it will not work anywhere near as well. The root cause needs to be addressed or more health issues will still be there. MDs are too quick to do surgery and should be more interested in the cause. And fixing it. Also bile dripping into the bile duct constantly can inflame the bile duct. And pre dispose people to bile duct cancer later on. Polyps can reduce in size if lifestyle habbits are changed. I appreciate that your perspective is a surgeons one. But surgery doesn’t solve anything. It should only be done if there is a risk of cancer. People need to realise that it took decades of eating bad and living bad to cause them. So it takes a long time to heal. Removal of gallbladders causes bile to drip continuously and this causes inflammation of bile ducts which predisposes people to cancer of bile ducts: Gallbladder surgery has been performed for around 140 years, with millions of operations each year. The great majority of people who have their gallbladders removed do not suffer any consequence, for a small number, there may be an initial fatty food-related loose stool that settles quickly. 3. Anyone who visits an emergency surgical department may attest to that. This is a dangerous and misleading assertion. In general, pain after meals that is situated in the right upper abdomen after meals (specifically dairy or fatty food) in the presence of suspected polyps, removal of gallbladder should be considered. Cholesterol deposits behave much like gallstones. There are a myriad other causes of abdominal pain, unrelated to gallbladder that requires careful assessment.

True gallbladder polyps are not affected by diet. They grow from the lining of the gallbladder and are not cholesterol or bile constituent related. Hence, there is no influence of diet on true gallbladder polyps. These will require surveillance or surgery because of the risk of malignancy, as explained in the video. Cholesterol deposits (which are not true polyps) may behave like gallstones, but once formed, diet will not affect these either. These have no malignant potential. There is no good way to differentiate between the two on scans. Please see the description for evidence.

My general doctor recommended surveillance and comeback in a year for a second ultra sound. I got a second opinion from a surgeon, and he recommended the same. The fact 9mm is so close to 10, it makes me nervous. I had an ultra sound about 5 years ago, and nothing. Over the past two years, my diet and stress level were very bad.

There are many causes of upper abdominal pain, which may not necessarily be gallbladder related. The causes include but are not limited to infection with bacteria (H Pylori), inflammation of the lining of stomach (gastritis) or (peptic) ulcer disease. No. The polyps are growths from the lining of the gallbladder. Just removing the polyp will leave fragments behind, and expose the patient to unnecessary risk.
True gallbladder polyps originate from the lining of the gallbladder and are not cholesterol or bile salt based. Diet will not influence the formation or progression of 'true polyps'. Since these have malignant potential, these ought to be followed up as suggested in the video. Cholesterol deposits are not 'true' polyps, i.e, these are much closer to small stones that are adherent to the gallbladder wall. These cannot be differentiated from small polyps. Cholesterol deposits do not have malignant potential but may behave like stones. Please see the evidence in the description.

Idk but I wanna blame my self because when I got my job before 6 months I was eating every single day like 1 or 2 times a day all the week unhealthy food Mcdo, fried chicken, pizzas, noodles, spaghetti oily, and not walking to much just 1.73 and my weight was 85 kg Then after I got constipated I visited my gastro and I requested for studios of ultrasound, x rayz colonoscopy and endoscopy in the ultrasound they found me a polyp in the wall gallbladder of 0.46 almost 5 mm. And then previous the exam I started to be more healthy avoiding this fast food from 85 I went to 83 k in 1 week. Doing fasting plus the scared and not sleeping, and then in 2 weeks I step to 76k after all this I bring up my weight to 79k and my skin look more healthy I look more skinny also in face because before my face was a bit chubby, and my digestion came back again, I wanna say if you are eating fast food and you didn't know how unhealthy is , stop it. During my endoscopy my gastro found a bacteria helicobacter pylori with erosive gastritis and yes Im taking the treatment for 14 days is already done and maybe I need to go by test of stoop to check if the bacteria is out ( I will say everything start once I went to the office of my job and I eat some food is also made by pork chicken beef, and some other things ) because after that I have constipation that's why I got all these test I was even doing well when is time to go to the bath but after going to the office the constipation started right now everything came back to normal And there is 3 things. I am a surgeon but I advise against surgery when it is not in the interest of patients and not supported by published evidence. In the end, it is the patient's choice what they wish to do, once given all the pertinent facts.
1. Polyps may reduce in size which is a function of diet:
It is important to understand that true gallbladder polyps (adenomatous polyps) are not affected by diet - period. These may be precursors of malignancy, hence the surveillance. An unhealthy diet is not the cause of true polyps.
2. Removal of gallbladders causes bile to drip continuously and this causes inflammation of bile ducts which predisposes people to cancer of bile ducts:
Gallbladder surgery has been performed for around 140 years, with millions of operations each year. There is NO evidence of an increased risk of biliary tract cancers after gallbladder removal. The great majority of people who have their gallbladders removed do not suffer any consequence, for a small number, there may be an initial fatty food-related loose stool that settles quickly.
3. Gallbladder surgery should only be done if there is a risk of cancer:
I admit patients daily with severe gallstone disease which may be a threat to their life all the time. Anyone who visits an emergency surgical department may attest to that. This is a dangerous and misleading assertion. Patients with symptomatic gallstones ought to consider the removal of the gallbladder.
4. For 'polyps' 6mm or smaller, with symptoms, there is a good chance that these are cholesterol deposits. These behave exactly like gallstones and are prone to the same complications (cholecystitis, cholangitis, acute pancreatitis). Further, there is no good scan to differentiate between 'cholesterol deposits' and 'true polyps'. It is also possible that smaller polyps may co-exist with cholesterol deposits in the same gallbladder. Hence, the advice to 'consider' gallbladder removal. Patients may choose to live with this condition, patients who do not have an operation after the first uncomplicated attack of pain, around 50% may not develop symptoms for some time. If no surgery is performed, surveillance is advisable.

Gallbladder is within normal in size with multiple fixed non shadowing echoes with range of 0.4 cm to 0.6 cm. Its wall is not thickened.

In general, pain after meals that is situated in the right upper abdomen after meals (specifically dairy or fatty food) in the presence of suspected polyps, removal of gallbladder should be considered. Cholesterol deposits behave much like gallstones.
There are a myriad other causes of abdominal pain, unrelated to gallbladder that requires careful assessment.

Hi Jiwan, As I explained in the video, gallbladder polyps less than 1cm may be observed unless there is pain or gallstones.

I just had my first ultrasound abdomen test since i am experiencing pain in right abdomen and also below the right lower rib. The result says my I have a polyp of 1.17cm. I am male and 32 yrs old. Should I have my gallbladder remove or should I wait 3 months and get ultrasound again?

My husband's gallbladder is normal in size.wall thickness is within limit (2.0 mm in thickness). There are multiple tiny echogenic structures adjacent the wall of gall bladder without producing any acoustic shadows bigger one measures about 6.6 mm in length.

I can only make general comments: Gallbladder polyps that cause symptoms ( such as pain after a fat rich meal) is a reasonable indication for gallbladder removal. There are many other causes of nausea which need to be investigated. Good luck!

True gallbladder polyps are not affected by diet. They grow from the lining of the gallbladder and are not cholesterol or bile constituent related.
Hence, there is no influence of diet on true gallbladder polyps. These will require surveillance or surgery because of the risk of malignancy, as explained in the video.
Cholesterol deposits (which are not true polyps) may behave like gallstones, but once formed, diet will not affect these either. These have no malignant potential.
There is no good way to differentiate between the two on scans. Please see the description for evidence.
@@thesurgicalscholar Thank you Kahn Klinik. I just had an ultra sound and they detected a couple of polyps. The largest was 9mm, so I am very nervous. My general doctor recommended surveillance and comeback in a year for a second ultra sound. I got a second opinion from a surgeon, and he recommended the same. The fact 9mm is so close to 10, it makes me nervous. I had an ultra sound about 5 years ago, and nothing. Over the past two years, my diet and stress level were very bad.
The following are indications for gallbladder removal:
An increase in size of polyp by >2mm during surveillance.
Symptoms (right sided abdominal pain - gallbladder pattern)
Indian ethnicity
Age >50 years
Presence of Primary Sclerosing Cholangitis (PSC).
Presence of gallstones and polyps.
Flat polyps (sessile) greater than 5mm at site of maximum thickness.

I have random pain in my upper abdomen especially when my stomach is empty and my polyp size is 7mm. It's been a long time since I have experienced pain though I have controlled my diet for long period. Is this all problem due to GB polyps?
It is uncommon for polyps to cause pain, however, gallbladder pain is typically after food, on the right and may move to the back.
There are many causes of upper abdominal pain, which may not necessarily be gallbladder related. The causes include but are not limited to infection with bacteria (H Pylori), inflammation of the lining of stomach (gastritis) or (peptic) ulcer disease.
Please always consult your doctor about your symptoms.

No. The polyps are growths from the lining of the gallbladder. Just removing the polyp will leave fragments behind, and expose the patient to unnecessary risk.

True gallbladder polyps originate from the lining of the gallbladder and are not cholesterol or bile salt based. Diet will not influence the formation or progression of 'true polyps'. Since these have malignant potential, these ought to be followed up as suggested in the video.
Cholesterol deposits are not 'true' polyps, i.e, these are much closer to small stones that are adherent to the gallbladder wall. These cannot be differentiated from small polyps. Cholesterol deposits do not have malignant potential but may behave like stones.

In general, small polyps are sometimes difficult to differentiate from cholesterol deposits. If there are no symptoms a follow-up ultrasound may be performed at six mostly intervals.
For patient with typical gallstone symptoms removal of gallbladder is advisable.

Idk but I wanna blame my self because when I got my job before 6 months I was eating every single day like 1 or 2 times a day all the week unhealthy food
Mcdo, fried chicken, pizzas, noodles, spaghetti oily, and not walking to much just 1.73 and my weight was 85 kg
Then after I got constipated I visited my gastro and I requested for studios of ultrasound, x rayz colonoscopy and endoscopy in the ultrasound they found me a polyp in the wall gallbladder of 0.46 almost 5 mm. And then previous the exam I started to be more healthy avoiding this fast food from 85 I went to 83 k in 1 week. Doing fasting plus the scared and not sleeping, and then in 2 weeks I step to 76k after all this I bring up my weight to 79k and my skin look more healthy I look more skinny also in face because before my face was a bit chubby, and my digestion came back again,
I wanna say if you are eating fast food and you didn't know how unhealthy is , stop it. During my endoscopy my gastro found a bacteria helicobacter pylori with erosive gastritis and yes Im taking the treatment for 14 days is already done and maybe I need to go by test of stoop to check if the bacteria is out ( I will say everything start once I went to the office of my job and I eat some food is also made by pork chicken beef, and some other things ) because after that I have constipation that's why I got all these test I was even doing well when is time to go to the bath but after going to the office the constipation started right now everything came back to normal
