It's been well proven that the promoters and organisers were to blame, the seeds were sown, Limp Bizkit were just doing their usual hype set much like this one, and the crowd was already pissed off.. so it would have happened anyway.

Yeah, "the monitors" literally refers to what Fred hears on the stage. The music can sound different on stage live with all the equipment near him, and he needs to hear guitar cues for vocals probably
