I mean really that goes for almost everything from the 50s and 60s, engineers suddenly decided that everything could be solved with nukes and nuclear power which led to some really hilarious and impractical ideas.
If this gets added to War Thunder, upon the tank's destruction, the reactor would contaminate the surrounding area and would slowly injure the crew of any vehicle within a certain radius of it, unless the vehicle is one of a handful tagged as having CBRN protection (pretty much exclusive to high-tier stuff without oscillating turrets).
Nah Gaijin should make it that if the tank get destroyed every tank and planes with a 10 mile radius also get destroy because the designer of the tank decided to put the nuclear reactor in the turret. Also tanks that enter the 10 mile radius after the explosion well have their crew weaken because radiation.

pretty much everything the us had that was in some way nuclear related had protection for the flash, bombers with special sunshades, some pilots (i think for the f-106, and other nuclear capable aircraft) would wear an eyepatch when deploying a nuke just in case the shades didn't work, they would be left with at least one good eye.
Here's a bit of funny pop culture for you... in Loki episode 5, right around the 37:30 minute mark, one of these tanks can be seen overgrown and partially buried, as well as damaged, but the shape is unmistakable and it's on screen enough it could be nothing else.

The #1 challenge of designing a viable nuclear-powered tank would be an efficient method of storing all that energy when not in use (AKA when you're not moving) as nuclear reactors are so slow to gain or decrease their output. You'd either need some amazing flywheels (could be directly attached to the engine drive shaft), a huge battery or a way to reliably piss away all that energy constantly. Challenge #2 would be an electric engine(s) capable of driving both the reactor setup and the tank itself which back in the day would have to be huge.. Today however, we're getting really good at creating micro nuclear reactors that can fart out as much as 20 MW and companies like Tesla are mass-producing 500+ horsepower electric engines with massive amounts of torque that only draw like 15-20 KW. Once you look at all the facts, it's really surprising fully electric tanks haven't been made standard yet.

Tank with a nuclear reactor seems crazy and unrealistic, but both USA and USSR had actual plane prototypes with nuclear reactors flying around at that time. Both counties thankfully abandoned those projects in early 60-s.

BWR would have spread radiations and created a steam explosion, but a MSR solved these problems when the USAF asked for a nuclear powered bomber. These reactor are wayyy safer than anything else.. in the end, the lobbying of the USN won and BWR and later EPR were the standard reactor design
About radiation danger if a reactor is build well there is no risk of radiation killing you and in fact the little ammout of radiation that escapes makes you build tolerance that makes you even more resistant to radiation so swaping crews would not be a necesity
