I know how that happened. Teachers have different hairstyle options than students, so Kyoshi's hairstyle ID is 7, just like Saki's. Her hair colour is Blonde, with is only available for her hairstyle, putting it on any other one makes it silver. And teachers weren't supposed to pop up in the basement back then, so you got a Saki with Marie Antoinette syndrome.

I found a cool/ sorta creepy myth.. 1. Go into the debug menu and use Ebola mode 2. Kill everyone, including the teachers and gym teacher except Midori Gurinuuuu 3. Go to the roof and Midori will not die when you go near her, she will just scream and a skull shaped mist will appear 4. If end her lifu with the knifu, she will die and kneel down before you ^-^ (Ps. I got this from prettygummybears video so credit where credit is due)

Maybe that mystery girl is just a glitch in the game's coding, looks just like Saki. It's not unusual for her to be there since she was the first mind slave, but I guess the glitch assumed it was her, I don't know. I'm just blabbing here.
The teacher's model is created by Saki's Model. It is not programmed for a teacher to get kidnapped. That's why it used one of it's models to create the teacher to Saki.

You could have just poisoned the bento, went and killed someone, disposed of it, and by the time you got back someone may have seen her and contacted a teacher. I think the main goal was to kill Kokona with poison and show yourself to the teacher, to find out if the teacher would accuse you of the murder even though poisoning someone doesn’t splatter you with blood
