The main distinction from what I have learned so far is that its the type of force. In spiral fracture you are getting a rotational/shear force, that will cause much greater angulation than an oblique fracture (which you can't see in the 2d image). An oblique fracture is more likely due to accidental trauma, whereas a spiral trauma (especially in young children) points towards intentional trauma.
Combined trauma, like an automobilistic accident... ex. you can have a lateral force causing a dyaphisis transverse fracture and a rotational force causing a transcervical fracture.
Why would we send he patient back and why would the wrist and should Xrays not have been taken at the sametime. I know many people different ways of doing things. Just a retorical question.

@7:16 transverse fracture of midshaft of femur. Distal segment is TILTED laterally to around ... degrees.
This is actually angulated medially.

Pavan Mehat from what I understand spiral fractures are more of curved lines whilst oblique are more straight

I thought one could assess whether it is open or closed if there is evidence of air infiltrate within the soft tissue on X-ray

You try to do the least amount of imagining as possible (minimize costs, and radiation exposure to the patient) if you can.
