Febrile Seizures or Febrile Convulsions for USMLE

  • Febrile Seizures are the most common seizure in childhood. It has a really good prognosis, there needs to be diagnosis of exclusion. Seizure rate increases during fever. Tends to occur in patients between the ages from 6 mo to 6 years. Familiy History and is associated with 19q. Clinically there is a rapid increase in temperature of greater than 39 degree celsius. Divided into simple and complex. Both of them are generalized tonic-clonic. Simple last less than 15 minutes and occurs once in 24 hours. Complex may have more movement during post-ictal stage. 30-50% may recur especially if they are less than 1 year old, low temperature, if there is family history and if it is complex.
    Work up you have to first determine the cause especially meningitis/encephalitis and if suspecting this you must first to Lumbar puncture. If less than 12 months it is more considered than if they are greater than 12 months.
    Must rule out herpetic meningitis and HHV6 which is associated with roseola. Then rule out metabolic such as hypoglycemia, electrolytes such as Calcium and Magenesium. Also grab a toxicology report. EEG helps rule out epilepsy but not always necessary. If there are atypical findings if then do some neuroimaging.
    Treatment is just reassure and educate the patient. Explain risk of recurrent. No long term anticonvulsants are given

    Also you can add midazolam bucal mucose or lorazopam intranasely .
    Prophilaxis is fenobarbiton intramuscular 8mg/kg and dose of diazepam 5 mg 10 kg. Dont use rectaly if they have diarrhea.
