  • Psychiatrist Guy Goodwin on the causes of bipolar disorder, why manic episodes are so dangerous and what medicines can be used to stabilise the mood.

    'Bipolar disorder affects in its more severe forms about almost one in a hundred people and in its milder forms it affects another one in a hundred people. So it’s pretty common: if we know about 200 people the chances are we know at least one person with bipolar disorder. '

    It's weird to say but my experience with Bipolar is pretty similar. The longest job I've ever had is 3 years. My mania can turn into extreme paranoia and the rage is unbelievable. When I get manic it only ends with me going to the hospital after becoming so crazy I know I need help. I have so much energy. I can stay up for days at a time. Sleep maybe a few minutes here and there. It's hard to build a solid foundation. I've lost relationships because of it. I've lost my education because of it. I'm afraid I can't pursue anything because the mania will sneak up and ruin everything. Even if it lasts a week. My behavior and lasting impression changes everything. I've lost his jobs. It's hard to tell if my depression is from my illness or my outlook in life. It's hard for me to see a life of success. I'm 36 this year. I was really good in school, but none of that matters if the mania shows up. The medicine isn't perfect. I've become manic on medication three times. I feel like even being hypomanic is dangerous because of how fast I can slip into hypermania. I try to be hopeful. It's hard to see a bright future.

      Im 37 and just found out too april 2021 . Its a scary thing but at least I know now. It's not a good tbh I've had all what you had gone through probably more. My advice get on medication and try groups that have same conditions . I wish i had something else like diabetes or anything but this . Just stay on the journey sometimes youtube or finding out answers online helps. If you ever get extreme anxiety . Try to tell yourself that there's nothing you can do about it at the moment . I found this helps quite a bit . The drugs work though . Get help though seek i mean . its only my first year . I know your pain . Its the only thing you can do . You are in control

    Lithium has proved effective for my longterm bi polar condition. Not the whole answer however. I think what I get now, after many years, is mixed states. Hardly drink now and lead a quiet life. I wondered if the pandemic would affect my condition and it has though I haven’t had much mania, thank god, but I suffered quite a major depression for a while and anxiety. That could be a situational reaction; perhaps my efforts to manage my condition have helped me get through the pandemic. I worked during one long period of remission and am very proud of running a small business for 15 years. I take in poetry readings. A life lived with care after diagnosis and with some abandon before.

    I was diagnosed at 15. I’ve been taking lithium on and off for years. My mania is absolute hell. I am angry and the rage scares me. It’s like I’m someone else completely. I never wanna go through it again. My lows hit me twice as hard and last twice as long as the manic episodes. Can’t move can’t pull myself out of bed. Im only capable of just existing. That’s it. Bipolar also causes memory issues. I sit in a fog more often than not. I can’t keep a job. My bipolar often gets in the way. Specifically my lows. Keeping to a ridged schedule dealing with the public when breathing is all I’m capable of doing. Lithium helps so much though. It’s helped with my depression and kept my manic episodes away. The biggest struggle I have is remembering to take it.

      @@JohnPChau mania is hell. I hate it with a passion. I skip hypo mania and derail into not sleeping paranoid and rage. Im sorry your struggling so hard with mania. I understand the paranoia. Seeing things that aren’t there. Hearing things. I should of been hospitalized a few times. But it has never been an option. Meds for bipolar is difficult to get right. Lithium is the only constant for me. I have periods where I need anxiety meds ect. My meds change with my mood. Don’t get discouraged. It takes time to find the right combination. Taking the meds constantly is half the battle. I set have an alarm set to take it every day. Because I don’t ever wanna experience the lows or highs again. It’s is exhausting to all be so extreme. Also get a mood tracker. I use one faithfully to. Helps me see if A high or low is possible. So I can put a stop to it before it happens. As for work. Your job needs to help accommodate. Bipolar also qualifies for disability. I’m currently fighting them and trying to get on it. 3 years is amazing to keep a job. I don’t think I’ve ever kept one longer than 6mo. I feel ashamed of it. I have the gene + many adverse childhood experiences.
    I was diagnosed at 24. I am 30 now, and I am dying :( I choose partners who are emotionally unavailable. It tortures me because my emotions are so strong, and when I’m feeling suicidal, they don’t care. But I have such a strong fear of abandonment that it is difficult to leave. I don’t know what to do. I worry I might do it one day because I just truly feel so alone. I drove off last night after a fight with my boyfriend and I texted him that these fights make me feel suicidal and he didn’t even respond. It exacerbates my condition and I just don’t know how much longer I can do this

      I’m so sorry you’re both going through this, I also have bipolar and many many traumas from my childhood as well. I was diagnosed in 2020 due to being suicidal. I was hospitalized two weeks. The first week was at a facility that kept me separate because I was over COVID but still testing positive. Took a month to test negative. This place made me worse and just pumped me full of drugs. The second place I went really really brought me back from that ledge. I didn’t truly want to die but all the things in my life and my bipolar collided and the dam I had up finally broke and flooded the mainlands. It took awhile to dry up. I’m still dumping water back in the dam. But I’m doing it one bucket at a time. That’s all we can do. We have to stay strong and know that we aren’t this illness. It’s easier said than done, I have experienced bipolar since I was a kid, my mom is bipolar and her father was, I’m super my sisters are undiagnosed bipolar as well. We need to change our thought pattern. We need to correct the needs and wants. We need routine, trust me it helps a lot.

    I find it hard to see how something can have genetic origins that starts mid-teens and often just disappears a decade later. Jordan Peterson's argument that it is complexity in a person's life that causes emotional problems that blow-out at the weakest point, which might be one's genetic make up, seems more convincing as mid-teens can easily get overwhelmingly complex and misery laden. However, this video is an excellent talk I'm most grateful to have heard.

      @Mickey Mouse Change in this world doesn't often come about through the sane, sober and solvent. The complexity argument seems good to me. Why do you reckon it's wrong?

    • @Wowzersdude-k5c
      @Wowzersdude-k5c 3 года назад +7

      It doesn't disappear a decade later. Where did you get that false info? Bipolar is almost always a life-long condition.
      Schizophrenia also presents for the first time in late teens or early 20's. It has also been shown to be highly genetic.
      Cardiovascular disease also has a genetic component and it doesn't show up until mid or later life.
      So I don't follow the argument that something that first presents in the early 20's can't be genetic.

    • @kharmalade544
      @kharmalade544 3 года назад +1

      @@Wowzersdude-k5c I'm not saying it does disappear as a matter of course; I'm saying it can and my authority for that is my own experience. I've no idea what percentage of sufferers experience it lifelong but I suspect it is the norm. But, if it can "just disappear", why is this so? That's why I feel genetic inheritance is a factor but not the determiner.

    • @Wowzersdude-k5c
      @Wowzersdude-k5c 3 года назад +1

      @@kharmalade544 Genetics are not the sole determinate as you said. However, we know that it tends to run in families. There's been a lot of studies of identical twins and we know if one twin has it, the other twin gets it about 70% of the time. Your risk goes up about 10 fold if one of your parents has it. But even then, your risk is only like 10% (much higher than chance, but by no means a foregone conclusion).
      The question is what are the environmental triggers? No one knows. But what we do know is the environment doesn't matter if you're not genetically susceptible. Genetics loads the gun, environment pulls the trigger as they say.
      Schizophrenia, like bipolar, is highly genetic. It seems there's been more research into schizophrenia regarding environmental triggers. The popular hypothesis now is that it is a strange autoimmune response in childhood that sets off some inflammatory cascade in the brain that results in the onset by the early 20's. There's also some evidence that it might develop in the placenta before the child is even born. Perhaps the mother got an infection during pregnancy that caused the placenta to develop abnormally.
      Also, schizophrenia and bipolar appear to be linked. We know that a lot of bipolar individuals have schizophrenia in their family, and we know schizophrenics often have bipolar in the family. I can attest to this from personal experience. A good friend of mine has bipolar 1 but his brother was a schizophrenic. Both of his parents also showed strong signs of mental illness but were never diagnosed.
      I am also bipolar 1. My brother is totally normal, but my father was completely crazy (never diagnosed with anything because he refused help). My mother is also on the bipolar spectrum (diagnosed by a shrink). My grandmother had severe depression - the kind where she lay in bed for years. My family is not unique, you'll see the same trend in tons of bipolar people's families.
      There's a clear genetic component here. I think this is established beyond a reasonable doubt. The real question is why do some people with the genes NOT get sick? We don't know, but it's a major area of research. My own opinion is we're going to discover it is the result of an autoimmune situation which facilitates inflammation in the brain. Perhaps brought on by simple infections.

      He is sharing his considerable knowledge on managing bipolar generally in a very short video. I don't think he needs to be bouncing around like a cartoon character on cocaine to do this effectively. I also did not hear him say anything to suggest there's no hope for people who get severe depressive eps/hypomania.
