But the eye definitely makes appearances, it's the personification of Sauron's Ainur. It's odd because it even seems to imply in Fellowship of the Ring that Sauron has a physical body again in Barad-dur because it's implied he is there for the torture of Gollum.

But along with that, it is hollow. It is unstable and brittle, and while it burns bright and hot, there is always an uncertainty to it, with no thought for tomorrow and no backup plan if things go wrong. When it fails, it will fail spectacularly, and crumble into dust.
Well that would make it quite unfit for a theme of mordor then. If there is one thing we know about sauron it's that he always thinks things through. His only mistake was underestimating his opponents reason and strength of will, apart from that it's all carefully planned strategy. For me this theme brings the thoughts of an overwhelming, to the point of bringing to insanity, evil power, until now hidden among the scariest of legends, just discovered to be worse then them

I believe he regains his physical form when Morgoth eventually comes back to Arda and prepares for Dagor Dagorath. He summons his full dark army including Sauron to fight for one final time.
