The Totem you're using gave you a 4th heal use. Most Totems and Carapaces have added benefits/deficits that aren't in their description. If you look on the far right side when examining a Carapace, Bracelet, or Totem, you can see its stats. Totems usually only affect Heal uses, but some affect other things.
