I think this is my first time seeing this one, sorry. Marisa is wearing her IN clothes here, so this was released in 2004 or later. The video quality looks like a typical video from the early days of Niconico though (2007+), so maybe it's not that old. The first fan-made animation likely predates Niconico, and it was probably uploaded to a temporary file host and posted on a message board or personal site, so it might be difficult to track down.

@@Wobuffet3 Edit: found a reupload of the arrangement (touhou jukebox) im still looking for the midi itself. i believe the video in question was just an arrangement of tabula rasa, the op claimed it to be the first 2hu arrangement. The arrangement in question is archived and stored in a recreation of the old site zun uses. I lost the link, the uploader was marisa nya.

Probably not. it's a lot easier to make a 4koma than a music arrangement. Some manga or other form of doujin work likely existed before this.
